Organisational Environment:

Australian Independent Living Enterprise Co-operative Ltd trading as Co-operative Home Care, is a Sydney based operation to deliver social care in a range of community settings. It is based on the fundamental premise that engaged employee owners deliver a higher quality of care and support than more traditional forms of home care provision. In 2016 we plan to grow from a single site co-operative home care provider to a multi-site group of employee owned home care providers, anchored to a network of neighbourhood based ‘Life Centres’ offering a range of health and social products and services to its local community.

Position Objective

  • To provide the best in home care service for frail, aged or disabled person.
  • Develop and implement case or individual plans for frail, aged or disabled persons
  • Implement and build upon existing supports in the person’s life(theinformal care network).
  • To build a network around each person, including the paid care team, and other professionals who are involved in the care and support of the individual (the formal care network).
  • To provide high quality care from a Human Rights Perspective.
  • To work with a small team of semi-autonomous support workers, to assist the team to manage the day-to day communications, day-to-day administration of service delivery, community engagement and data functions.

Requirements of the Job

  • Coordinating and supporting the Care Team involved in the delivery of the case or individual plans.
  • To have a thorough understanding of person centred care principles and resources and tools.
  • Managing communication with both internal and external stakeholders.
  • Developing and maintaining functional and supporting relationships with Co-operative Home Care customers in order to promote positive outcomes.
  • Ensuring compliance with all internal policies and procedures, statutory and contractual requirements in relation to service delivery.

Key Responsibilities

  • In conjunction with Operations Manager , managereferrals new customer inquiries
  • In consultation with administration officer, create quotes and individual budgets for customers.
  • Conduct holistic assessment for new service users.
  • Set goals with customer and/or their carer.
  • Create Support Plan that includes goals, timeframes and outcomes
  • File and store data
  • In consultation with HR, assist teamswith online platforms
  • In conjunction with teams conduct assessment monitoring and review of customers as required.
  • Conduct regular Quality of Life reviews for customers as required.
  • Liaise with GP, other medical specialists, Allied Health professionals and other community organisations.
  • Maintain professional relationship with customers, family members and carers.
  • Promote business in local community, to increase word of mouth referrals
  • Maintain tasks register ( schedules , meetings, forums, workshops)
  • Attend community meetings, forums, workshops as require.
  • Plan and organises monthly Care Team meetings in allocated area.
  • Provide information and encourage healthy behaviour for both the staff and the service user.
  • Support teams in their personal and professional development.
  • In consultation with Operations Manager, conduct staff training on operations process, computer programs etc
  • Monitor team’s compliance with company requirements including mandatory, training and WHS training.
  • Risk Management includingmaintain incident register, complaints register etc.
  • Ensure confidentiality in accordance with professional standards and agency policy.
  • Participate in agency Continuous Quality Improvement activities.
  • In times of staff absences provide operational assistance to ensure continued operation and efficiency

Selection Criteria

  • Have a strong understanding and empathy for the needs and requirements of people who are elderly or who have a disability or serious medical condition.
  • Have a history of providing high quality service in a similar role.
  • Demonstrate superior customer service, interpersonal, communication and administration skills.
  • Possess a high attention to detail.
  • Work well in a fast paced and energetic team environment.
  • Be capable of working part time hours from 8.30m to 6.00pm and additional on call duties.

Skills & Experience

  • A minimum of a Certificate IV in Community Services or a tertiary degree in a health related field.
  • A satisfactory Police Check or the ability to acquire one.
  • A current, valid driver's licence.
  • Must have full working rights in Australia.
  • Excellent communication skills, both written and verbal.
  • Ability to work autonomously.
  • Excellent organisational skills.
  • Excellent time management skills.
  • Computer literate with access to own computer and mobile phone.
  • A genuine interest in the holistic care of frail, aged and disabled people who live in the community.
