Clinician/Case Manager

Lamp Community ends homelessness of Los Angeles' most vulnerable individuals, primarily adults with mental illness, through a continuum of services and housing, enabling them to reach their highest level of self-sufficiency and community integration.

As part of a multi-disciplinary team, the VASH Clinician/Case Manager will travel to multiple locations in the greater Los Angeles area to meet, transport, and work with Veterans to attain and sustain permanent housing in partnership with the Veterans Administration (VA). The Clinician’s primary responsibilities are to outreach and engage homeless Veterans and to coordinate housing, mental health, primary health, and recovery services that will ensure housing stability and community integration.

ESSENTIAL DUTIES & RESPONSIBILITIES

1.  In collaboration with the VA, outreach to chronically homeless Veterans with the objective of engaging individuals to determine eligibility for permanent housing

2.  Complete pre-screenings and intakes with Veterans to submit to the VA in accordance with all VA Policies and Procedures

3.  Assist Veterans complete their VASH Section 8 application ensuring all required documentation is obtained and submitted with the application

4.  Attend all Public Housing Agencies appointments with Veterans

5.  Ensure that all Veterans with VASH Section 8 vouchers find an appropriate apartment within 21 days of receiving their voucher

6.  Work with and contact Property Managers and Landlords to set up appointments to show Veterans different apartment options

7.  Travel to multiple housing locations in the greater Los Angeles area (including Lancaster/Palmdale) to provide case management

8.  Coordinate and implement mental health and recovery support services with individual Veterans (caseload of 20-25)

9.  Coordinate with the VA to ensure Veterans are connected to primary health care, mental health and recovery support services

10.  Develop effective, trusting relationships with Veterans, with a focus on facilitating independence and maintenance of improved physical and mental health

11.  Collaborate with each Veteran to develop their individual service plan; review and update it within VASH required time lines

12.  Assist management and other Clinicians with program evaluation and modification for future program improvements and growth

13.  Assist Veterans with becoming involved in daily activities, socialization, health maintenance, sense of community, and progress in recovery reflective of Lamp Community’s Harm Reduction Model which emphasizes customer choice, self-determination and psycho-social rehabilitation

14.  Work cooperatively and cohesively with other members of the VASH team, including participation in weekly staff meetings, group clinical supervision, and trainings and provide clinical consult to other VASH advocates as needed

15.  Develop and maintain a complete, accurate, and current case file with all required documents from the VA and Lamp

16.  Assure that all Veteran’s paperwork and data collection is complete and up-to-date in the data management system

17.  Adhere to National Association of Social Workers’ Code of Ethics

QUALIFICATIONS

1.  Employment contingent on passing the Veterans Administration background check and security clearance process

2.  MSW required; ASW registration with CA BBS preferred

3.  Minimum of two years of experience working with homeless individuals, preferably Veterans

4.  Experience working with homeless individuals with mental illness and substance abuse diagnosis

5.  Current, valid California Driver’s License with an acceptable driving record and reliable vehicle

6.  Outstanding clinical and strong team-building skills

7.  Detail oriented with excellent time management, organizational, written, verbal, and computer skills

8.  Able to obtain and maintain CPR/1st Aid certification

9.  Minimum 35 WPM typing speed preferred
