Core Purpose / To engage with the community about the Burger Centre programs including:
  • Client referrals and access to services
  • Volunteering support at the centre
  • Community liaison
  • Family connection

Key Responsibilities
  1. Client assessment and liaison
/
  • Respond to initial enquiries regarding The Burger Centre
  • Assess clients in their homes prior to attending, where necessary refer to Social Worker for home visit
  • Arrange and support potential clients coming to the centre
  • Liaise with clients’ families
  • Manage referrals to and from My Aged Care Portal
  • Ongoing monitoring of clients including the documentation and liaison with case workers, family members and other relevant stakeholders
  • Identify and support carers
  • Facilitate groups with clients and volunteers as required
  • Attend team meetings as required

  1. Volunteer Support

  • Support volunteers while at The Burger Centre
  • Provide in-service trainings as required to volunteers and staff in consultation with Volunteer Managers and other relevant training providers
  • Maintain linkage with JewishCare, Montefiore and volunteer managers and other volunteering organisations

  1. Community Engagement
/
  • Attend relevant community forums to promote Burger Centre programs
  • Presentations to community groups and at professional forums on Burger Centre
  • Attendance at relevant professional forums for Eastern Sydney area
  • Participate in development of marketing material

  1. Be familiar with legislation relevant to the performance of the role.

  • Actively participate in professional supervision anddevelopment
  • Attend mandatory training on an annual basis, including but not limited to Elder Abuse, WHS, Fire and Evacuation

  1. Administrative duties
/
  • MaintainClient Management system implemented by the Burger Centre
  • Liaise with Transport Coordinator and Administration Coordinator with regards to new clients
  • Undertake and be responsible for a variety of administrative tasks relating to the position

Qualification Knowledge and Experience
Essential / Desirable
Qualifications: /
  • Recognized Degree or Diploma in Community Management, Adult Education or Leisure and Health
  • Current unrestricted NSW driver’s license and willingness to drive own car
/
  • Training in group work
  • Presentation and public speaking skills

Experience /
  • Experience with working with clients with high and complex needs including dementia
  • Experience in working in multidisciplinary team

Knowledge: /
  • Demonstrated knowledge of aged care, in particular knowledge of dementia and related conditions.
  • Knowledge and experience of client assessment
  • Knowledge and experience of individual service plans and their implementation
  • Working knowledge of Consumer directed care model
  • Experience in Commonwealth Home Support Program
  • Demonstrated knowledge of external services provided by the wider community
  • Knowledge of food safety requirements
/
  • Knowledge of an Eastern European language e.g. Russian, Polish, Hungarian, Yiddish
  • .
  • Understanding of Jewish values, heritage and culture

  • Excellent written and verbal communication skills

  • Demonstrated computer skills including the use of social media
