Sponsoring an International Visiting Intern
Courtesy Appointments
Forms for DS-2019: Office of International Student and Scholar Services (ISSS)
Résumé/Curriculum Vitae
- The Intern sends a copy of his or her résumé or curriculum vitae to sponsoring department by email or fax.
- The Intern also sends a color copy of their passport photo to the sponsoring department by email (preferably as a PDF document).
Letter of Invitation to Intern - Original signed by Dr. Petrie & Intern
- Department sends the draft of the invitation letter to Gyll for review. The draft should follow the model which can be obtained electronically from Gyll. The draft must meet the chair’s approval.
- All Interns must consent to a criminal background check. This should be stated in the letter.
- Gyll approves or suggests changes to letter.
- Gyll forwards approved draft to Mari, who prepares letter on Dr. Petrie’s letterhead with her signature.
- Mari forwards letter for signature to Intern for signature.
- The returned original letter with Intern’s signature is kept with Mari until the International Packet for the DS-2019 with all the forms is ready.
Courtesy Personnel Data Sheet (required for background check)
- The Courtesy Personnel Data Sheet is available electronically at: http://www.fau.edu/provost/files/courtesydataform_revised.pdf
- The Intern fills out and signs the form and sends it by postal mail to Mari.
3.Whenever we send a Personal Data Form to a person who is going on a courtesy appointment, we must include the form at the following link. The last category on page 2, “Other,” applies to courtesy appointees:

Request for DS-2019
1.Request for DS-2019 is available electronically at: http://www.fau.edu/isss/forms/ISSS_EVP_SCHOLAR_REQUEST.pdf
- Sponsoring Professor fills out Part I, inserts the start and end dates and provides necessary documents.
- Visiting Intern fills out Part II, provides the necessary documents and returns the forms/documents to Mari.
2.The Sponsoring Professor will sign and return to Mari who will seek the sponsoring department Chair’s signature and Dr. Petrie signature.
3.Mari keeps the form until the International Packet for the DS-2019 is ready.
International Packet for DS-2019 (ISSS)
- The packet, with the following mentioned forms and signatures, is sent to Mari, Room 526:
- Copy of the invitation letter signed by Dr. Petrie & the Intern,
- Courtesy Personnel Data Sheet,
- One (1) original Bank Letter (Financial Evidence) from the Intern,
- Department Request for DS-2019 completely filled out and signed by Sponsoring Professor, Department Chair, and Dr. Maria Petrie
Processing for DS-2019
- Mari hand-carries and delivers packet to the International Student and Scholar Services for processing.
- Please allow approximately 60 Days for ISSS to process the forms.
- The Intern must schedule an appointment with home country’s embassy or consulate to obtain J Visa.
J Visa
- Mari picks up the DS-2019 packet from ISSS.
- Mari emails a scanned copy of the DS-2019 to the Intern.
- Mari sends the original DS-2019 to the Intern via Express Mail.
- The Intern must bring a copy of an original Bank Letter and DS-2019 Packet to the home country’s embassy or consulate at time of appointment.
- The sponsoring department must notify the ISSS Office of any delays or changes such as name spelling immediately.
Personnel Action Form (PAF)-(only required if the Intern stays for more than 90 days) http://www.fau.edu/provost/forms.php
- The sponsoring department prepares the PAF, gathers the appropriate signatures, and sends it to Gyll Rowan, with a copy to Mari.
- The PAF must have the following attachments:
- Courtesy Personnel Data Sheet Packet
- Signed Offer Letter
- Résumé
- Sponsoring department will request criminal background check with HR.
