- Parents: initial next to one of the release times on each day you are approving your child to sign out. *Students will NOT be released during testing time.
- Return this form to the front office no later than Monday, December 14. If a student has not returned this form by the deadline, notes/emails to sign out will NOT be accepted during exam week; parents must come into the school to sign out.

I also recognize that my student is required to take any and all exams they are not exempt from and if they miss an exam they were to take that grade will be entered as a zero.”
