Change ofOwnership Form

Complete this form to change the ownership of a decoder. The completed form and other required documents can be sent via email to , faxed to 011 577 4901 or dropped off at a MultiChoice Service Centre or Agencies.

The following documents are required to complete the change of ownership:

  • Completed change of ownership form
  • Proof of valid identification of the new owner of the decoder
  • If bought from a second hand retailer the proof of purchase is also required
  • If previous owner is deceased, and the new owner is related to the deceased, we also require a letter from the Executor of the deceased estate or a letter of Authority (Should credit be transferred please include it in the letter)
  • If previous owner is deceased and not known to the new owner, we also require the proof of purchase

Transfer of Credit to the New Owner

I ______(name and surname) with ID number ______hereby instruct MultiChoice to transfer the nett credit from my MultiChoice account as at the date of transfer of the ownership of the decoder to ______(name and surname of the new owner).

I hereby indemnify MultiChoice against any claim that may arise as a result of the transfer of the said credit.

We require a proof of identification of the previous owner.
