Central Iowa Aquatics
September 3, 2009 will be the first day of practice for the Fall Season. To provide assistance with registration, CIA will have a registration table September 3-4 (Thursday and Friday) at Urbandale and/or Central Campus.
Swim Groups
Central Iowa Aquatics (CIA) is a USA year-around competitive swim team designed to improve the level of competitive swimming in the central Iowa area. CIA offers five different practice groups as well as a masters swim program. For a description of each swim group please see the CIA web site at www.centralowaaquatics.org
2009-2010 Swim Year
The swimming year is divided into four seasons as outlined below. The practice schedules for each swim group are posted on the CIA website at www.centraliowaaquatics.org. Practices are subject to change. Practice cancellations due to weather, pool closures, or other conditions will not be rescheduled. Payment Options
· Annual: This option is available only in April, with payment due by April 30, 2009. Annual fees are discounted 5%.
· Seasonal: Paying the seasonal fee requires a commitment to swim the entire season. Payment is due at the beginning of each season during which a swimmer participates.
· Monthly: Monthly fees are for those swimmers who cannot commit to an entire season or who prefer a monthly payment. The monthly fee is due the first day of each month that a swimmer participates. Payments are not collected in March or August.
· USA registration fee is due at registration. All Age Group, Senior Group, and High School swimmers must pay this fee, even if they do not swim in USAS meets. The registration fee is not required for the Intro or Masters swim groups, unless they swim in USAS meets.
· During the summer season, the City of Des Moines charges CIA $70 per swimmer for the use of Ashworth Pool. This $70 fee is included in the annual and summer season fees, but not in the monthly fees. Monthly payers for June and/or July will be required to pay this $70 fee in addition to their regular monthly fees.

*HS Boys who pay either the seasonal fee for Fall or the monthly fee for November are entitled to swim the month of February after their high school season finishes.
** Monthly summer swimmers pay a $70 City of Des Moines surcharge for the use of Ashworth Pool, in addition to their monthly fees. This surcharge is included in the annual and summer seasonal fees.
New Swimmer Discount
CIA offers a discount for swimmers new to the club. These swimmers receive 50% off the monthly rate for the first two consecutive months that they practice with CIA. In order to qualify for this discount the swimmer must not have swum with CIA at any time in the past. This discount can only be used once. If a swimmer participates for one month and then quits, he/she does not qualify for an additional discounted month at a later date.
High School Stroke Session
High school swimmers have the option to practice with Coach Dave during their high school season. These sessions are held once weekly at Central Campus, following the end of late practice. The fee is $100 for any high school swimmer who has not paid an annual fee. High school swimmers who practice and/or compete with CIA during their high school season must have approval from their high school coach and/or athletic department.
Payment Policy
· CIA registration fees must be paid prior to participation in CIA activities.
· Swimmers without a signed and paid USA Swimming registration are prohibited from entering the water for insurance purposes. This must be done on a yearly basis. (Intro and Masters swimmers are exempt from this policy unless they compete in USAS meets).
· All payments are due on the first of the month. A 15-day grace period is provided. A $5 late fee is assessed after the 15th. After 30 days, the swimmer will not be able to participate in CIA activities until all past fees are paid plus $25.

Financial Assistance
The CIA parent board tries to assist families with financial needs. However, limited funds are available. For additional details, please contact the head coach, Dave Joensen, or the business manager, Bill Colville.
Family Discount
Families with three or more swimmers receive a discount. The registration fee for the third swimmer and additional swimmers beyond three is reduced by 50%. This reduction applies to the lowest registration fees.
Refund of CIA Registration Fees
· USAS registration is not refundable.
· Monthly fees are not refundable.
· Refunds of the annual registration fee and seasonal fee will be prorated based on the monthly rate.
· There will be no refund of fees for swimmers participating in the new swimmer discount.

Participation Agreement and Waiver/Release of Liability
Please read carefully before signing. This is both a participation agreement and a release of liability and waiver of certain legal rights.
The enrolled participant and the parent/guardian of the participant agree and understand that swimming is a hazardous activity. I recognize that there are risks inherent in the sport of swimming, included but not limited to, paralyzing injuries and death.
The participant hereby agrees to indemnify and hold harmless CIA, its coaches, officers, directors, agents, and employees against any liability resulting from any injury that may occur to the participant while participating in any of the swim programs offered by CIA. The participant also agrees to indemnify CIA for any damages incurred arising from any claims, demand, action or cause of action by the participant.
The participant authorizes any representative of CIA to have the participant treated in any medical emergency during their participation in any of the swim programs offered by CIA. Further, the participant and/or parent/guardian agree to pay all costs associated with medical care and transportation for the participant.
I have noted on the back of this form and notified the head coach of any medical/health problems of which the staff should be aware of.
