PRACTICE COMMITMENT
Our goal is to enable patient’s engagement, in providing and continually offering quality care for all, and to encourage shared decision making between patients, GP’s and clinicians at all times.
SURGERY ACCESS
All our sites are easily accessible from all the main road routes, bus and train stations. (Please see our website for location maps)
ACCESS FOR THE DISABLED
The Practice has access for wheelchair users and disabled toilet facilities are available. We also have lift access to services on the first floor at our Culcheth site and all sites have disabled parking.
HOURS OF OPENING
Monday - Friday 08.00-18.30
We offer an extend surgery hours till 20.00 at our Culcheth site on a Tuesday.
Patients from any of our sites can attend these sessions
In order to ensure continuing education for our staff we allocate the last Thursday afternoon of every month as protected learning time. On such occasions we are closed.
MAKING AN APPOINTMENT
To make an appointment:-
- Phone the surgery
- Speak to reception
You may see any Doctor at any of the CCA Care Partnership sites.
We offer a text messaging service to all our patients to ensure you do not forget your appointment. If you would like to take advantage of this service, please provide your mobile phone number to our reception staff. You will then automatically receive confirmation and reminders.
Please remember to let us know if you are unable to attend for a booked appointment every missed appointment is a cost to the NHS
HOW TO REGISTER
If you move into the practice area registration forms are available on our website or at reception for your completion.
TELEPHONE CONSULTATIONS
We offer a comprehensive telephone consultation service if you feel that your medical problem can be dealt with over the telephone, please talk to our reception team who will arrange for a Doctor to call you.
DISPENSING
Each of our sites has a pharmacy alongside the surgery to facilitate rapid dispensing of your prescription. The pharmacy will also provide advice and can provide over the counter medicines.

HOME VISITS
If you really feel too unwell to come to the surgery, please call before 9.00am to book a home visit. Home visits take up a great deal of time, so we do ask, whenever possible please try to come to the surgery
PATIENT CHOICE
We always respect your rights as a patient to express a preference in regard to choice of practitioner; however there may be times where your preference cannot be met due to sudden illness or same day appointments. On these occasions we ask for your understanding and co-operation.
CHOOSE & BOOK
Choose and Book is a service to allow you to choose your hospital or clinic and book your first appointment.
When you and your GP agree that you need an appointment, you can choose which hospital or clinic you go to, the date and time of your appointment.
If you are unable to deal with this yourself or need support in making your appointment, please call our referrals team, and they will organise your appointment for you
OUT OF HOURS
Should you require URGENT assistance when the surgery is closed you should follow the process detailed below:-
FIRST PORT OF CALL: NHS 111
Call 111 to reach a team of highly trained advisers who are also supported by experienced nurses. Once through on your call the adviser will ask questions to help assess your symptoms, give you advice or redirect you to an alternative appropriate service.
NEXT STEP FOR URGENT NEED – THE WARRINGTON OUT OF HOURS SERVICE

FOR A MAJOR EMERGENCY
You should call 999. This might include chest pains and/or shortness of breath as an example.
REPEAT PRESCRIPTIONS
There are many ways you can request a repeat prescription:-
Tear off the slip of your prescription and post it to us or drop it off
at the surgery, or why not make it easier and visit our website, click
on“repeat prescriptions” and fill out the online form.
Please allow 2 full working days for your prescription to be processed once it has been requested.
CERTIFICATION FOR ILLNESS
You do not need a certificate for your employer if you are absent from work through illness for seven days or less, you can use a self-certification form (SC2) for the first week which you can get from the website or at reception, after the first week you will need to make an appointment to see a doctor for a Med3 form.
TRAVEL VACCINATIONS
If you require any vaccinations relating to foreign travel you will need to make an appointment with a practice nurse to discuss your travel arrangements. This will include which countries and areas within countries you are visiting to determine the vaccinations required. You should ensure you leave sufficient preparation time in advance of having any travel vaccinations.
SUGGESTIONS & COMPLAINTS
We always welcome feedback and suggestions that you may have to help us improve the service we provide at our surgeries. If you wish to make a complaint, please could you write to the Practice Manager who will deal with it under our complaints procedure.
RIGHTS & RESPONSIBILITIES
In order to provide adequate & informed care for you we are obliged to keep records. This is increasingly done using computers. We comply with the Date Protection Act 1998 and other guidance on privacy and data confidentiality and we take this very seriously.
In order to manage services and improve the quality of care we provide we may need to share some information on practice activity e.g. with the Department of Health and other NHS bodies. Whenever possible this information is anonymised i.e. names and other identifying details are removed.
Information is NOT shared with any third party outside the Health Service (e.g. Insurance, employer, solicitor) without your explicit consent and agreement. We are obliged by law to provide certain information e.g. notification of certain infectious diseases.
Please be aware that CCA Care Partnership takes a zero tolerance approach to any patient or visitor who verbally or physically abuses staff members.

PATIENT VOICES GROUP
At CCA we have an active and growing Patient Voices Group. The aim of this group is to ensure patient evolvement and engagement in regard to services provided including what, where, when and of how we ensure quality at all times. We will always welcome new members and would love some younger members of our practice to get involved.
