JOB TITLE:Lead Case Manager

PROGRAM:Therapeutic Foster Care

JOB SUMMARY:

The purpose of the Therapeutic Foster Care Program is to provide a planned, intensive system of clinical and supportive services for children with emotional and behavioral disturbances in a foster family. It also serves the purpose of providing children the opportunity to learn to function within a family setting prior to permanent placement. The final aspect is to provide non-institutional treatment in a structured environment for children with low to severe emotional disturbances.

The Case Manager is responsible for: 1) Supervising TFC foster family case managers, 2)Providing case management services for foster families. 3) Serving as a member of the treatment team to plan, develop, coordinate, monitor and evaluate care as designed by multi-disciplinary team for children and therapeutic foster families. 4) Participate in program development and evaluation. 4) Develop and implement service plans.

REQUIREMENTS:

  1. Adhere to TDFPS Minimum Standards, OSHA, Level of Care Monitoring Standards and other regulating/accrediting bodies.
  2. Provide case management services for foster families.
  3. Coordinate child’s admission by working with DFPS or other agency caseworker and foster family, preparing child’s file, preparing agreements/forms, being present at placement.
  4. Attend ARD meetings to assist school personnel.
  5. Maintain current child’s records by filing plans, reviews, criticalincidents, etc., at least weekly.
  6. Maintain child’s daily log files.
  7. Notify foster parents of child’s needed medical/dental appointments.
  8. Coordinate monies for clothing and camp.
  9. Act as liaison between TDFPS and foster families.
  10. Transport clients as needed.
  11. Maintain foster family files by filing in-service forms/materials, updateslicensing materials, etc., at least monthly.
  12. Conduct home-visits twice monthly.
  13. Assist foster parents in obtaining in-services training in the community.
  14. Attend monthly supervision group.

3. Serve as a member of treatment team to plan, develop, coordinate, implement, monitor, and evaluate care as designed by multi-disciplinary team for childrenand foster families.

4. Assist in program development and evaluation.

5. Schedule treatment and medication reviews.

6. Arrange psychological and psychiatric testing as needed.

7. Provided crisis intervention in all foster homes as needed.

QUALIFICATIONS:

Master’s Degree in Social Work or related human service field; or a Bachelor’s Degree in Social Work or related field and at least two years experience working with children and/or families having problems of adaptation as a result of abuse/neglect. CPS background a plus.
