Case Planner Education Details

Report BackgroundThe Case Planner Education Details report can help supervisors and managers of Case Plannersmonitorthe tracked children in open Family Services Stages (FSS) withan education recordin CONNECTIONS.

Report PurposeProvides a detailed list of tracked children in open FSS Stages that have education information entered in CONNECTIONS. The report is only applicable to districts and agencies with a Case Planner role in stages containing education data.

Target UsersManagers and supervisors of Case Planners from Local Districts and Voluntary Agencies

PromptsUpon opening the report, users are asked to select one value from a list of choices for the following report prompt:

  • Case Planner Agency: Select the name of the Local District or Voluntary Agency for which you want to run the report. Districts and VAs are listed alphabetically by agency code. Agencies appear at the prompt only if they have a Case Planner role in at least one open FSS Stage containing an education record for a tracked child.

NotesFields are not grouped in orderto facilitate exporting to Excel for sorting; the output is pre-sorted by Case Planner Agency, Site Code, and Unit Number, Case Planner Name, Case ID, Stage ID, Child Name, School Year of Last Record Created, and Date Late Updated.

Data Item / Data Definition
Case Planner Agency / The 3-digit agency code and name selected at the report prompt.
Total Number of Children with Education Record / The number of tracked children in open FSS stagesin CONNECTIONS with any information entered on the Education tab. This is a distinct count of the PID.
Total Number of Stages with Education Record / The number of open FSS stages in CONNECTIONS with any information entered on the Education tab. This is a distinct count of the Stage ID.
Report Date / The date on which the report was run.
Data as of date / The latest refresh date of the OCFS Data Warehouse – that is, the last time data was received from CONNECTIONS. Data for this report is refreshed every other week.
CP Agency / The 3-digit agency code for the Case Planner assigned to the stage.
CP Site / The 3-digit site code for the Case Planner assigned to the stage.
CP Unit / The unit number for the Case Planner assigned to the stage.
CP Name / The full name of the Case Planner assigned to the stage.
Case ID / A unique integer which identifies the case.
Stage ID / A unique integer which identifies the stage.
Stage Name / The name of the stage corresponding to the Stage ID.
WMS Case ID / The case number used in the WMS system.
Jurisdiction / The 3-digit code of the jurisdiction, or county,with responsibility for the stage.
Stage Sub-Type / The 3-digit code value to further define the stage type. Values may include:
  • CWS (Child Welfare Services)
  • CCR (Child Case Record)
  • ADVPO (Advocate Preventive-Only)
  • ICPC (Interstate Compact for the Placement of Children)
  • OTI (Out of Town Inquiries)

CM Agency / The 3-digit agency code for the Case Manager assigned to the stage.
CM Name / The full name of the Case Manager assigned to the stage.
Associated Worker Agency / The 3-digit agency code for worker that is “associated” with, or linked to, specifictracked children in the stage.
Associated Worker Name / The full name of worker that is “associated” with, or linked to, specific tracked children in the stage.
Child PID / The CONNECTIONS Person ID for the tracked child.
Child CIN / The WMS Client Identification Number for the tracked child, if available in CONNECTIONS.
Child Name / The full name of the tracked child.
Child DOB / The date of birth of the tracked child.
School Year / The most recent school year for the tracked child, based on the most recent Date Effective (see definition below). If multiple school years were entered in CONNECTIONS with the same Date Effective, they are all included in the report.
Student ID / The NYS Department of Education Student ID for the tracked child, if available in CONNECTIONS.
Date Effective / The most recent Effective Date entered by the worker on the CONNECTIONS Education tab. This is the date on which education information on the tab is applicable.
Date Last Updated / The date that information on the Education tab was most recently updated for the tracked child.
Grade Level / The most recent grade level for the tracked child, based on the most recent Date Effective (see definition above). If multiple grade levels were entered in CONNECTIONS with the same Date Effective, they are all included in the report.
