Case Manager: College and Transitions

Position Overview: The Case Manager: College and Transitionsprovides case management support, with an emphasis on post-secondary education placement, retention and completion. This position is responsible for leading post-secondary readiness classes,preparing Corpsmembers for post-secondary education, and following up to ensure successful retention. This position is also responsible for maintaining required documentation and data input for various grants supporting the program.

Areas of Responsibility:

Program Planning, Implementation and Evaluation

Oversee, implement, and ensure continuous improvement of a college preparation program, both during and after direct MHYC programming

  • Develop and implement college preparation workshops and classes to be facilitated within Career Readiness Training and career development classes at MHYC
  • Ensure that youth are prepared for, and connected with, the appropriate educational opportunities and post-secondary education institutions, including colleges, vocational and trade schools
  • Facilitate the completion of FAFSA for participants, as well as placement testing, college applications, and essays
  • Facilitate a minimum of quarterly review meetings with all MHYC program graduates in post-secondary placement
  • Successfully coordinate campus introduction programming with Community College of Denver

Transition and Post-Program Opportunities

  • Implement a standardized transition plan for all assigned Corpsmembers in collaboration with the case management team
  • Implement a systematic placement effort, working with local education and training institutions to provide post-program opportunities for Corpsmembers and alumni in coordination with the case management team
  • Maintain follow-up for all YouthBuild graduates in post-program placements in accordance with agency standards
  • Meet regularly with representatives from post-secondary institutions to review our Alumni success in their post-program placement and identify other supports as needed
  • Maintaina resource library of education programs, scholarship opportunities, post-secondary educational opportunities and institutions accepting AmeriCorps Education awards. Provide Corpsmembers and students with an overview on how to use their AmeriCorps award post-MHYC

Liaison with Post-Secondary Institutions

  • Cultivate and develop relationships with college representatives at Community College of Denver and Emily Griffith Technical College, as well as other identified institutions (community colleges, apprenticeship programs, certification programs, etc.), including, but not limited to, the financial aid office, academic advising offices, administrative offices, First Generation and other college support programs, and other appropriate contacts within each institution to meet the needs of Corpsmembers
  • Attend community-networking meetings and trainings related to post-secondary education, accessing funding for post-secondary education, Corpsmember development, and placement. Identify organizations and connections appropriate for increased involvement or placement opportunities for Corpsmembers

Corpsmember Development

  • Develop relationships with Corpsmembers as well as an understanding of their outside supportsto identify resource needs. Communicate these needs within the team to provide the most effective service delivery possible
  • Work with Corpsmembers and graduates to proactively identify potential barriers and develop plans to address barriers for successful PSE completion
  • Maintain appropriate relationships with outside supports, including probation and parole officers, other case managers or support providers to ensure continuity of support

Administrative Duties

  • Trackand enter data in variety of systems that track program and post-program implementation.Coordinate and maintain Corpsmember files, including obtaining all necessary documentation in accordance with various grant requirements.
  • Other duties may be assigned as necessary

Program Support

  • Support and lead the YouthBuild team in the implementation of a college-going culture
  • Work closely with the Case Management team to ensure maximum benefits and opportunities to link Corpsmembers with educational and employment opportunities

Qualifications

Education: Bachelor’s degree in career counseling, psychology, sociology or a related field. Significant professional experience in a related field may be substituted for post-secondary education

Experience

  • Experience working with a diverse group of people in a team atmosphere
  • Minimum three years working as a case manager or a related position with responsibility for managing the documentation for multiple funding streams and oversight of 50+ youth on caseload
  • Experience with classroom facilitation; preferably with ages 18-24
  • Minimum one year experience implementing post-secondary education transition and preparation programs preferred.

  • Ability to pass a pre-employment background check and drug test. Drug testing may also be required during employment. May be subject to FBI Background Check, which includes fingerprinting
  • Valid driver’s license with insurable driving record
  • Knowledge of Denver Metro Area preferred
  • This position has recurring access to vulnerable populations

Hours & Compensation

This is a full-time regular non-exempt position with benefits. The salary range for this position is $32,000-$36,000 depending on experience. Hours may vary based on Corpsmemberneed, butare typicallyMonday-Thursday 8:00-4:30 and Friday 8-4. Some evenings and weekend hours may be required for Corps related functions.
