Complete and submit this Proforma in order for your activity to be considered for accreditation with CaRST.

Before beginning, please refer to the CaRST Accreditation – Rules and Conditions on the CaRST website to assess whether your activity is eligible. We also encourage you to contact the CaRST team while planning your submission to ensure the activity aligns with the aims of the program.

Throughout this Proforma, instructions and guidance are provided in red text. Complete the Proforma by replacing the red text with the details relevant to the specified activity.Refer to the completed Proforma for the course ‘EndNote: For beginner’ as an example of the information that is required.

Before submitting make sure that:

  • all red text and inapplicable options have been deleted;
  • all information in the Proforma (excepting URLs) are in black text
  • superfluous spacing has been removed and the formatting is of an acceptable standard (compare to the completed Proforma for the course ‘EndNote: For beginner’

Provide the completed Proforma to your Postgraduate Coordinator/Head of School for endorsement;

Email the completed Proforma and the School's endorsement to or return to the Adelaide Graduate Centre in hard copy.

Evidence of Completion / A condition of activity accreditation is that the organiser must provide the student with evidence of completion, which the student can then upload to their ePortfolio.
