Proposal for Modifications of an Existing Programme
FORM A (Module Titles & Assessments)
The following form encompasses modifications to existing module titles and assessments, where the change does not significantly impact the wider programme validation. For all other modifications, please use Form B (see Appendix 1). Seek advice from the Faculty Director of Quality if you have any uncertainty about which form to use. Please NOTE:
- ALL modifications must be discussed and approved first by the appropriate programme committee (or equivalent body).
- Changes can only be made to future runs of modules. This process cannot be used for a retrospective change.
- Proposals for changes that are likely to have an impact upon programme or level/stage learning outcomes must be made on Form B.

A Word version of the updated Programme Specification (obtained from QSO) with all new modifications clearly markedusing track changes so they are immediately apparent to the reader.
All modifications to the programme approved before 1 November 2014 should be attached as an annex to the Programme Specification. If you do not have a WORD copy of your programme specificationplease contact the Quality and Standards Office.
Complete the tracking sheet below summarising the modification and attach to the final page of the Programme Specification. Please consult your FDQ before providing this list.

Appendix 1 – Summary of which form/process to use
for modifications to a programme

(NB if additional resources are needed then the planning form will need to be submitted to Faculty Programme Planning Executive before final approval of the modification)
Faculty Quality Committee has the discretion to require a validation event to be held.

- Introduction of a new pathway or route
- Changes to taught doctoral programmes
- Changes requiring PRSB re-approval

FPPE/ASC recommends either small-scale or full validation

[1] If you have any questions about this, please contact Data Management () to confirm which programmes would be impacted.
[2] For changes to multiple module assessments you can still use one form. Simply insert rows to list all the modules and attach a separate document outlining the changes. PLEASE NOTE: if your programme is regulated by a PRSB you should consult your Faculty Director of Quality regarding multiple assessment changes and the processes required for this.
