PURPOSE: The purpose of this policy is to assist the Calhoun County School Wellness Program (SWP) nurses that have a Telehealth unit remotely connect with the Calhoun County Public Health Department (CCPHD) practitioners in a Child & Adolescent Health Center (CAHC).
POLICY: School Nurses with a Telehealth unit will triage students that present to the health office and determine if a Telehealth visit to the CAHC provider is necessary based on the nursing assessment.
PROCEDURE
- SWP Nurses will assess and treat students that have a written consent for treatment on file.
- SWP Nurses will consider a Telehealth consult if the assessment indicates services needed outside the RN scope of practice.
- Parent/Guardian will be contacted by the SWP nurse to discuss a Telehealth consult option, however, if unable to reach the parent/guardian, Telehealth consult may proceed based on written consent.
- Students will complete the Sliding Fee Scale (SFS) form for billing purposes.
- Consent and SFS will be scanned and sent in a secure manner electronically to CAHC staff for inclusion in Electronic Health Record (EHR).
- SWP nurse will document clinical findings in the SWP electronic health record and indicate Telehealth visit.
- CAHC staff will register the patient in the CAHC EHR and complete all necessary clinical and billing documentation per CAHC procedures.
Use of the Telehealth unit shall be limited to connecting with the CAHC provider for evaluation
