FOOTBALL/CHEERLEADING UNIFORM INFORMATION
Practice and/or game uniforms are included in the registration fee. Football uniforms consist of helmet, shoulder pads, practice and game pants, socks, and game jersey. Cheerleading uniforms consist of a skirt, shell, hoodie, and/or Warm Ups. The game uniform for Football and Cheerleading are rentals and will be turned in at the end of the season. A fee will be charged if the uniform is not turned in. Your child will be able to keep the practice jersey and socks for football and sweatshirt for cheerleaders. Your athlete will require the following items: Football: Cleats, Athletic Cup; Cheerleading: spanx/bloomers (must be purchased from BRYC - $10), white long sleeve T, white short sleeve T shirt, white socks and white athletic shoes.
EQUIPMENT HANDOUT
By signing below, I agree that I have been issued the items above. I understand the equipment/uniform issued is my responsibility and will care for it using the instructions provided. I understand that all equipment items must be returned in the condition they were issued, with the exception of normal wear and tear. I agree to inform my coach immediately if my issued equipment becomes damaged or lost. I understand that if I do not return the equipment I was issued as indicated above, I will be charged replacement fees according to the schedule below. I understand the BRYC will make two (2) attempts to recover my equipment and may then pursue collection proceedings, and that I will be responsible for all costs incurred, including collection and litigation fees.

CODE OF CONDUCT & DISCIPLINARY POLICY
BRYC Football and Cheer Code of Conduct
All participants in Braddock Road Youth Club activities (administrators, players, coaches, officials, and spectators) are expected to conduct themselves in accordance with the principles of good sportsmanship and appropriate behavior and will comply with all BRYC policies, rules, and procedures.This Code of Conduct applies at practices, games, and all other BRYC-sanctioned activities.
Participants will:
· Exemplify behavior that reflects good sportsmanship, respect, courtesy, and a positive attitude.
· Maintain self-control at all times.
· Refrain from using abusive language or profanity, making humiliating remarks, gestures of ill temper, verbal or physical threats, and/or physical assault upon another player, coach, official, or spectator.
· Not engage in any behavior which would endanger the health, safety, or well-being of any player, coach, official, or spectator.
· Treat players, coaches, officials, and spectators with respect regardless of race, creed, color, national origin, sex, sexual orientation, religion, or ability.
· Follow field/facility rules and regulations, respecting at all times the property of others.
· Not use alcohol, illegal drugs, or tobacco products, or carry any firearm or weapon during any event in which BRYC is a participant.
Anyone who violates this Code of Conduct is subject to immediate dismissal from any BRYC event and disciplinary action up to and including being expelled from all Braddock Road Youth Club activities.Alleged violations of the Code of Conduct shall be referred to the appropriate Sport Commissioner for investigation, to include interviewing the parties involved.
In the event of a violation of the Code of Conduct, disciplinary action shall include, but not be limited to, the following, in any order or combination:
1. Verbal warning issued by a BRYC Sport or Club official.
2. Written warning issued by a BRYC Sport or Club official.
3. Suspension or immediate ejection from a sports event issued by a BRYC Sport, Club, or event official, with written documentation of the incident kept on file.
4. Suspension from multiple sports events issued by a BRYC Sport or Club official, with written documentation of the incident kept on file.
5. Season suspension or multiple season suspension issued by the Club, with written documentation of the incident kept on file.
6. Permanent suspension from all Club activities issued by the Club, with written documentation of the incident kept on file.
Appeals of disciplinary actions will follow the appeals process outlined in the BRYC Risk Management Policy.
Individual BRYC sports may adopt additional policies and procedures relevant to their unique sport’s activities so long as they do not conflict with these Club-wide policies.
The BRYC Code of Conduct and Disciplinary Policy will be made available to individuals and be posted on the BRYC Sport and Club websites.

PARENTAL CONSENT
I, the PARENT/GUARDIAN of the above-mentioned candidate for a position on a BRYC Football Team or Cheerleading Squad, hereby give my approval to his/her participation in any and all BRYC Football or Cheerleading activities during the current season. I assume all risks and hazards incidental to such participation including transportation to and from all activities and do hereby waive, release, absolve, indemnify, and agree to hold harmless, the local BRYC Football and Cheerleading Organization, the respective FCYFL Association and the Braddock Road Youth Club, the organizers, sponsors, supervisors, participants, and persons transporting my child to and from activities, for any claim arising out of injury to my child.
INSURANCE
The organization has Accident Insurance coverage for medical and hospital expenses, with a give deductible amount for each accident incurred. The conference Insurance is considered as secondary coverage, when there is any other valid and collectible coverage provided by the parent’s separate insurance. Maximum coverage is provided for any one accident with a give maximum dental coverage for sound, natural teeth. In executing the foregoing release, I the undersigned hereby acknowledge and represent that (A) I understand that any claim for medical service which arises out of injury must be reported to the team WITHIN TWENTY (20) days of the date of injury (B) I have read the foregoing release and understand it, and sign it voluntarily. I understand that any Registration Fee or other sums paid does not constitute a direct premium payment for insurance.

Uniforms should be washed immediately after each use if possible.
Pre-Wash
· Turn garments inside out
· Remove pants pads if applicable
· Heavily soiled garments should be pre-treated.
o Any pre-treatment brand found at your local grocery store is fine as long as it does not contain bleach.
o For blood and grass stains, soak in cold water before washing
o An oxygen based laundry booster can be used as well
Washing
· Use a high water level to minimize mechanical action on uniforms
· Wash in COLD WATER only
· Do not overload washer
· Do not wash with other items, especially towels or linens
· Do not use Chlorine Bleach or Fabric Softener
· Do not Dry Clean
Drying
· Remove uniforms immediately after washing
· Hang Dry – DO NOT PUT IN THE DRYER
o If you must use the dryer, use the damp or air dry setting. The lower the setting, the better for the uniform High heat can cause shrinking, cracking in the ink and printing, wrinkling of the twill
o Do not overload the dryer – this will cause items with ink printing to stick together
o Remove from dryer immediately to prevent wrinkling
· DO NOT IRON
Storing
Store uniforms in a cool dry place, protected from direct sunlight and fluorescent lighting.
HELMETS, SHOULDER PADS, PANTS PADS
· Allow to air dry after practice / games before putting them back in your bag
· DO NOT PUT IN WASHER
· DO NOT DRY CLEAN
· DO NOT SOAK / SUBMERGE IN WATER/CLEANING AGENTS
· Helmets (inside only), Pants pads and shoulder pads can be wiped down with disinfecting wipes
· Shoulder pads can be sprayed lightly with Lysol or Febreeze.
· Allow to completely air dry before wearing.
TIP: Spray cleats and tennis shoes with disinfecting air freshener and stuff with newspaper. Let sit overnight in a cool dry place. This removes the odors.
