Position: Hospice Medical Records Coordinator
Team / Reports To: CFO-COO
Description and responsibilities
1. Establishes Patient Charts and enters referral intake data, newly admitted patient information, patient demographic updates.
2. Ensures all admission documentation is properly signed and dated, including physician authorizations.
3. Handles requests for medical records and hospital inpatient charts.
4. Serves as DME procurement specialist, inclusive of Suncoast (EMR) entries.
5. Provides Admissions and HOS Nursing Directors weekly report with outstanding claims issues updates, coordinating inputs from Contracts Specialist and Clinicians.
6. Reviews all Pharmacy invoices for accuracy and works any issues with vendor/s before giving to Manager for approval.
7. Responsible for post discharge patient file audits, ensuring all necessary documentation is complete and filed appropriately.
8. Participates in reception coverage relief rotation as needed.
Qualifications
1. BA or BS degree from a nationally accredited school, preferably in Public Health, or Health/Business Administration preferred.
2. Minimum 5 years of work experience or three years in a hospital, healthcare or non-profit environment.
3. Outstanding organization skills, flexibility and cooperation in performing job responsibilities.
4. Excellent computing skills including experience with Enterprise software, MS Office
5. Positive disposition and calm demeanor.
6. Ability to deal tactfully with clients, caregivers and the community.
7. Self starter.

Commensurate with experience; medical and dental insurance coverage, Paid Time Off, 401(k) with employer’s matching contributions.
