APPLICATION FOR WATER SERVICE
(effective January 09, 2013 per Ordinance 2013-1)

The undersigned, being the owner
or authorize agent of the property located at
hereby makes application for water service for the above described property which is used as a Residence.
The Consumer Rate for water is as follows (note subject to change as ordinance is updated):
up to 1"$ 50.00 plus usage rate of $3.00 per 1000 gallons
up to 1 ½"$ 113.00 plus usage rate of $3.00 per 1000 gallons
up to 2"$ 200.00 plus usage rate of $3.00 per 1000 gallons
up to 2 ½"$ 326.00 plus usage rate of $3.00 per 1000 gallons
up to 3"$ 450.00 plus usage rate of $3.00 per 1000 gallons
The undersigned agrees to pay the Water Department for the installation of water service. This payment will be used for connection and installation fees to curb line of property. This service charge applies as follows (note subject to change as ordinance is updated or as charged by contractor). Charges for service begin at the time the service is installed and are due within 40 days of billing. A late fee of 5% will be charged on any balance due after the 40 day billing period.

The undersigned further agrees to comply with the rules and regulations of the Water Department for the use of water, and pay for same in accordance with the rule, regulations and rates established by an Ordinance of the Borough of Elmer and agrees that the Water Department shall have the right to amend and alter said rules, regulations and rates at any time. A late fee of 5% will be charged on any balance due following the last day of the quarter due.
This application when accepted by the Water Department shall be deemed a contract between the Water Department and the applicant and shall be for the period of one year from the first day of the quarter in which this application is made. This contract will continue from year to year by implication on payment of charges unless thirty days notice of the desire to terminate the contract be given by either the Department or the consumer.
