Birthday Party Rental Agreement

Our birthday party packagereservations are taken on a first-come, first-served basis. Before submitting this form, we suggest you contact the Bridge office to check availability. Once you have confirmed that one of the party time-slots is available, please complete this form and return to The Bridgeat the address listed above, along with your deposit. BIRTHDAY PARTY RENTAL RULES AND GUIDELINES:

Renters and guests must remain in the specified party area(s) until time of departure. If you or your guest would like a tour of The Bridge, please contact the front desk to set up an appointment.

There must be one (1) adult present for every ten (10) children. Toddlers must be supervised by parent or guardian.

Children must be supervised at all times.

Maximum time allowed for parties is two (2) hours unless otherwise noted. If the event time exceedsthe terms of the rental agreement, the Renterwill be charged $25 per half hour.

If you have more party guests than specified on the rental agreement, you will be charged an additional fee of $8.00/per child charge ($12 for Arty’s Ultimate Pkg). If fewer than the guaranteed number of guests attend, the renter will be responsible for the full guaranteed number (specified on this rental agreement). (PLEASE NOTIFY US OF ANY FOOD ALLERGIES OF THE PARTY CHILD OR GUESTS).

No staples, tacks, pins, or nails may be used to affix decorations. No adhesive tape of any kind can be used on the meeting room walls, furniture or carpet. In the event that proper authorization is not received and damage results, the cost of repair and/or replacement will be billed to you.

If you choose to bring your own decorations, you will have 30 minutes before your event reservation time to set-up decorations, and 30 minutes after your reservation time to clean-up. Renter is responsible for installing and removing all decorations during the designated times.

Set up specifications and times should be discussed with and approved by The Bridge staff at least 2 weeks before rental date. Details must befinalized no less than seven (7) business days before the date of the event.

Please note, party packages only include enough supplies for the number of children specified with package choice. Should party supplies be needed for adult attendees, additional fees will apply.

Helium balloons are not permitted. You are responsible for informing party guests of this policy. Should balloons be brought into the facility, there will be a $10 per balloon removal charge if let loose in The Bridge facility. Aerosol/ Spray string is not permitted.

Except for cakes/cupcakes, no outside food or beverage is allowed.

Any items left in the room after renter leaves will be discarded. Please be sure you have all belongings before departure.

Use of The Bridge parking lot is included in the facility rental. The Bridge is not responsible for tickets incurred by guests parking in “NoParking” or “Handicapped Parking” areas.

1 Month advance notice is required for all rentals, and a $35non-refundabledeposit is required to book a party slot within 48 hours of response from Sales Manager .Full payment is due on the day of the event (cash, money order or credit/debit only).

Renter may change rental date or time if at least two (2) weeks advance notice is given with availability, however deposits are good for only 6 monthsfrom date of agreement.

Rental Dates and Times are subject to availability.

RESPONSIBILITY AND SAFETY GUIDELINES:

Renter is responsible for the behavior of all party guests— including any damages incurred during their stay at The Bridge.

Renter and guests are expected to respect the property— excessive noise, running, vandalism, alcohol,smoking, and any type of unsafe behavior is not permitted. Anyone found disobeying these rules will be askedto leave the property.

All party guests using playground equipment or Tuscany Lanesmust sign an Xtreme Kidz waiver prior to entering the Playzone area.

Groups and individuals who use the meeting rooms are guests of The Bridge, LLC, and use of the space should reflect that understanding. You agree to comply with all applicable qualifications, rules, policies and procedures as determined by The Bridge, LLC Administration, and all federal and state laws and standards. You may obtain a full copy of all Bridge policies and procedures from the front desk, or we will forward a copy to you electronically.

PLEASE NOTE: Your party is not officially reserved until we have received your deposit and you receive a confirmation email or verbal confirmation is given from The Bridge staff. We look forward to helping you create an Out of This World birthday experience!
