The undersigned, herein after called the “APPLICANT” hereby applies for membership in the Benguet Electric Cooperative, Inc., hereinafter referred to as the “COOPERATIVE”, and in consideration for acceptance of this application do hereby state the following:

TERMS AND CONDITIONS

1.  The APPLICANT will pay to the COOPERATIVE the sum of FIVE PESOS (5.00) ONLY, upon acceptance and approval of the application by the COOPERATIVE, which will constitute as the APPLICANT’s membership fee.

2.  The APPLICANT will, when electric energy becomes available, purchase from the COOPERATIVE all electric energy needed in his/her premises and will pay the corresponding bill computed based on monthly rates to be determined from time to time in accordance with the by-laws of the COOPERATIVE.

3.  The APPLICANT will cause his/her premises to be wired in accordance with wiring specifications approved by the COOPERATIVE.

4.  The APPLICANT, upon becoming a member, assumes no personal liability or responsibility for nay debts. Liabilities of the COOPERATIVE. It is expressly understood that under the law, his private properties is exempt from execution for any debts/liabilities of the COOPERATIVE.

5.  The APPLICANT will grant the COOPERATIVE, at its request the necessary tights and easements to construct, operate, repair and perpetually maintain on the property owned by the undersigned and in or upon all roads, streets or highways abutting and deliver to the COOPERATIVE any conveyance or instrument to which the COOPERATIVE shall deem necessary or convenient for said purpose. All lines, kilowatt-hour meters, switches and other appliances and/or equipments constructed or installed by the COOPERATIVE shall remain its sole property and therefore shall have the right to access to said properties.

6.  The APPLICANT shall have the rights and privileges granted to members under the Charter and by-laws of the COOPERATIVE or any amendments thereto. Further, the APPLICANT shall comply with and be bonded by such by-laws and Rules and Regulations which may from time to time be adopted by the Board of Directors.

7.  The APPLICANT, prior to the actual electrical connection, will sign the COOPERATIVE’s contract for Electrical Service Connection and shall further be bound by the provisions thereof.

8.  The acceptance of this application by the COOPERATIVE shall constitute an agreement between the APPLICANT and the COOPERATIVE. The agreement shall continue to be in force for one (1) year form the date service is made available by the COOPERATIVE to the APPLICANT until cancelled and/or revoked by either party by giving SIXTY (60) DAYS prior notice.
