(Name of Nursing Facility)is committed to providing you top quality care during your stay. As part of this commitment, we are participating in a four year project to reduce, if possible,the number of times you have to go to the hospital. The purpose of this project is to look at new approaches for care and payment to allow us to provide you with the best possible care at the right time and in the right place. This project is sponsored by the Greater New York Hospital Association (GNYHA) Foundation and the Centers for Medicare Medicaid Services (CMS).

Facts about what we are doing

The project provides enhanced services at the nursing home in order to reduce the number of times residents have to go to the hospital. The nursing home and your doctor may receive additional payments from CMS for using these on-site services to help you get better in your nursing home while getting the same treatment as would be given in the hospital. The focus of the additional payments will be treatment for residents with Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD) or asthma, dehydration, pneumonia, skin infections, or Urinary Tract Infection (UTI). The services include:

Providing enhanced on-site services and support to keep you out of the hospital with:

  • A visit by a doctor, nurse practitioner, or physician assistant after a problem is identified;
  • Oxygen therapy to help you breathe more comfortably;
  • Fluids that are given intravenously (IV)or under your skin so you feel better faster;
  • Medications including antibiotics;
  • Complex wound dressing changes;
  • Additional nursing services including lab and diagnostic tests;

Coordinating yearly and as needed care conferences to discuss your goals of care;

The project also supportsthe nursing home and its staff with education and resources to improve the quality of every resident’s care.

What this means to you

There are no additional costs.

You do not need to change doctors or insurance.

Your benefits and coverage will not change.

You may receive enhanced services to help manage your care at the nursing home instead of going to the hospital.

You may choose not to receive these additional services at any time.

Protecting your privacy

Your privacy will be protected. As a part of this new program, we will be collecting your personal and health information in order tounderstand how well the program is working. This information will be kept private. The GNYHA Foundation(and the Nursing Facility Name)follows Federal and State laws that protect the privacy of personal and health information.
