Bay-Wise Signs Order Form

As of 2015, we now have larger signs for non-residential sites. The two sizes are in landscape orientation with 4 holes drilled (one in each corner).

Complete this order form and send it to me. When your order is filled, you will receive an invoice. Please pay when you receive your signs. Make your check out to: University of Maryland.

NOTE: To complete this order form, place cursor anywhere inside of the table and double click. The table will shift slightly as it is highlighted. Ignore that shift. Now place the cursor in the cell you want to change in the “# Requested” column and type in the amount of the item you are requesting. Use the up and down arrows to move from one cell to the other.Hit the down arrow after your last entry. The table will automatically calculate the amount you owe. When done, place cursor outside of the table and click once on your mouse.Click on first gray block and begin typing.
