  • Ask mother for the date of her scheduled postpartum visit with her healthcare provider. If she is unable to provide the date, support her in making the appropriate appointment.

  • Collect Substance Use and Smoking Inventory

Parenting

  • Collect the Infant Birth Form (if this is first visit with family)
