Ø  Medical Insurance: AETNA High Deductible Health Plan

§  Monthly Pre-tax Employee Contribution

·  Family $358.00 per month

§  Annual Deductible

·  In-Network - $1500 / Individual

·  In-Network - $3000 / Family EE+1

·  Out-of-Network - $3000 / Individual

·  Out-of-Network - $6000 / Family EE+1

§  HDHP - All covered expenses including prescription drugs accumulate toward annual Deductible. Deductible must be met prior to benefits being payable per Co-insurance and Co-pay

§  Co-insurance: 85/15, 60/40

§  Rx Co-pays: $10 / $25 / $50 (mail order available)

§  Most Preventative care 100% with Deductible waived

Ø  Vision Care: VSP Choice Network

§  Deductible is $50 / Person per calendar year

§  Preventative care is covered at 100%

§  Co-insurances on other levels of care at 80% or 50%

Ø  Health Savings Account: PayFlex

§  Creates a bank account in your name for you to save money to pay for out of pocket medical expenses, tax free

§  Funds are always yours and roll over each year

§  Requires enrollment in the Awana Medical HDHP

Ø  Group Life Insurance: CIGNA

§  Coverage equals one times the employee’s annual salary

§  Voluntary Life coverage available, paid for by the employee

Ø  Short Term Disability / Long Term Disability: CIGNA

§  STD is self insured through Awana on an accrual basis

§  LTD coverage with Cigna after 1 year of service

Ø  Vacation

§  Less than 5 years of continuous employment, 10 days

§  Between 5-9 years of continuous employment, 15 days

§  10+ years of continuous employment, 20 days

§  New full-time employees will be advanced a pro-rated allowance for their use during the year in which they are hired

Ø  Sick/Personal Days

§  8 days per year, 2 of which may be used as personal days

Ø  Holidays

§  New Year’s Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving , the Friday after Thanksgiving, Christmas Eve, and Christmas Day

§  Awana is closed December 26-December 31st

Ø  Additional Paid time Off

Ø  401(k) Retirement

§  Automatically enrolled at 5% of your salary after four months of employment and 21 years of age

§  Pre-tax contributions up to 60% of base salary (plus Catch-up if eligible)

§  100% vested at the time of enrollment

§  Employer match of 25% of your deferred contribution, up to a maximum of 3% of your eligible compensation

(3% of salary x .25% = .75% Maximum Employer Match)
