·  If instructions are given to “Please see Mrs. Hoffman in the Counseling Center,” write your name at the top of the chart and return it to Mrs. Hoffman.

·  The required paperwork will be delivered to you in class.

·  Must be a female, living with a parent or guardian, unmarried and without children
·  Must be willing and able to serve as a volunteer for a minimum of 100 hours throughout the year
