o  In Patient and Out-patient treatment for TBI-must be referred by VA Case Manager (must be VA eligible)

o  TBI Support Group free and open

o  Texas Department of Assistance and Rehab Services- (non VA insured) alternative funding source

o  Have VA CM call Patience

·  Freedom Office Supplies-“Select Veterans” Clay Weger

o  Launched Spring 2015

o  Office supply providers

o  Veteran assigned agent number to buyers and able to get 5% percentage of funds and 500 cards with agent ID

o  Discuss agencies utilizing source for office supplies

·  September H0H Outreach Preliminary Report

o  80 volunteers

o  6-7 CA new veterans

o  8 updates to current CA

o  Several outreach/BOLO list contacts and reconnections

o  One family reunification

o  Great kits

o  Including family members for opportunities

·  Goal for Functional Zero and Mayor’s Challenge by Veteran’s Day

o  Getting our waitlist to zero in 7 weeks (details in staffing discussion)

o  Spreading case management intensity amongst program staff

o  Idea to break out waitlist by programs by numbers

SSVF Caritas hired new case manager-first day is Monday Sarah Lubeck

CM and LOS-Wed-Friday (Military Conference Training)
