2. Assist student to identify symptoms/behaviors that are disruptive to the classroom process.

2. Student will identify and utilize resources in school to assist him in managing his symptoms and behaviors. / 1. Collect data regarding student academic progress and behavior in school – initial and ongoing.
2. Assist teachers and parents/guardians to provide consistent structure and clear expectations.
3. Educate school staff regarding student expectations and classroom interventions to manage symptoms and behavior, including preferential seating, alternative ways to complete assignments, individualized behavior modification plans, and token reward systems.
4. Student will come to the nurse’s office for supervised administration of the following medication (s) according to the medication authorization form signed by MD. / School Nurse/classroom teacher - ongoing
