ATTENTION VISA APPLICANTS
(OTHER THAN STUDY)
Before submitting your file, take a moment to review it thoroughly to be sure it is in conformity with the list of requirements .
For your convenience, use this addendum.
- APPLICATION FORM
- make sure it is duly completed and legible(better if printed/typed). You should sing it in front of the consular officer.
- PHOTOGRAPH: (No staplers and/or glue are available at the Visa officer window)
- make sure it is ALREADY ATTACHED on the “photo space”: of your Application Form and it is passport size (full face and front view).
- VALID PASSPORT
- make sure you have one photocopy of its first page (only the page with the photo and personal data) and the page with your US valid VISA (if you hold one).
- PROOF OF LEGAL RESIDENCE IN THE JURISDICTION
- make sure it is submitted strictly as specified in the list of requirements.
E.PERMANENT RESIDENT CARD/DRIVERS LIC. and / or STATE ID
- Make sure it issubmitted along with a photocopy of each one.
F.TRAVEL ITINERARY
- make sure you have the exact flight information (round trip), it must be FINAL meaning you cannot change it after your application is submitted.
- PROOF OF LODGING
- make sure you have the confirmed Hotel reservations or invitation letter
as specified in the list of requirements.
- PROOF OF FINANCIAL MEANS
- make sure your most recent checking account balance is a Bank original.
Remember :YOU MUST HAVE SUFFICIENT FUNDS for your entire stay in Italy.
I.LETTER OF YOUR EMPLOYER
- make sure it is in original and his/her signature certified by a Notary Public.
make sure your salary is shown on monthly or annual basis, not hours;
- if you are self-employed: make sure to add the documents specified in the list of req.
J.PROOF OF OVERSEAS HEALTH/ACCIDENT INSURANCE
- make sure you have one simple official certificate from the Insurance Company
indicating that your specific plan will fully cover you while in Italy.
(Where doctors and/or hospitals will be paid directly).
- PAYMENT (VISA FEE IS NOT REFUNDABLE)
No personal checks – no cash – no credit cards.
- make sure your MONEY ORDER or CERTIFIED CASHIER CHECK is “ready”
(with the exact amount payable to the Italian Consulate in Chicago and your signature).
L.RETURN ENVELOPE (NO FEDEX – UPS – ETC.).
- make sure your USPS EXPRESS MAIL is properly PRE-PAID and with both addresses FROM /TO already completed and relative label properly attached.
Please apply as soon as possible, according to your schedule, bearing in mind that we can not accept application submitted more than 3 months before the date of departure.
IF YOU FEEL THAT YOUR FILE IS “LATE” AND/OR ONLY“PARTIALLY READY”, IT WILL BE USELESS TO SUBMIT IT TO THE CONSULAR AGENT.
