Submissions may be withdrawn, modified, and resubmitted prior to the formal bid opening due date. Any submission modification(s) submitted after the “Bid Opening Due Date” will not be considered.
The County reserves the right to accept or reject any or all Bids, or any part of any Bid, and to waive any informalities or irregularities in the Bid.
All costs incurred by the company to respond to this solicitation may be wholly the responsibility of the Bidder. All copies and contents of the Bid, attachments, and explanations thereto submitted in response to this ITB, except copyrighted material, shall become the property of the Jefferson County Commission regardless of the Consultant selected. Response to this solicitation does not constitute an agreement between the Bidder and the County.
The County is not responsible for delays occasioned by the U.S. Postal Service, the internal mail delivery system of the County, or any other means of delivery employed by the Bidder. Similarly, the County is not responsible for, and will not open, any bid responses which are received later than the date and time indicated above. Sealed bids marked ITB 130-16 "IMAGING EQUIPMENT & SERVICES" will be received by the Purchasing Manager, Room 830 * 716 Richard Arrington Jr Blvd N, Birmingham, Alabama.
Bids will be accepted until 11:59 P.M. central time (standard or daylight savings time, as applicable) on WEDNESDAY, SEPTEMBER 28, 2016. Bids submitted after this date and time will not be considered.
Bids will be publicly opened at 10:00AM on THURSDAY, SEPTEMBER 29, 2016.
STATEMENT OF PURPOSE
This document outlines the bid specifications for Cooper Green Mercy Health Services Invitation to Bid to update imaging systems in the Radiology Department. Project plans call for replacement of obsolete equipment with state of the art technology, relocation of some existing general radiographic equipment, and service agreements to support all purchased equipment. CGMHS requests an all-inclusive service that includes equipment, installation, service agreement, removal and disposal of existing obsolete equipment, physicist inspection and report submissions to Alabama Department of Public Health Department of Radiation Control upon project completion.
Bid should include the following:
· Removal/disposal of obsolete equipment
· Equipment costs, including installation and warranty information
· “New” or “Remanufactured late model” equipment will be considered. Please submit bid with total cost to purchase all “new” equipment; list pricing for “remanufactured late model” equipment as a separate line item option.
· Cost of service agreements (years 2-5 if warranty covers 1 year) including labor, parts, glassware, remote support and software updates
· Costs related to connectivity to CGMHS’ Merge PACS if applicable
· Application training for specified equipment.
BACKGROUND
Cooper Green Mercy Health Services’ Imaging Department offers diagnostic services for our Clinic and Urgent Care patients. Our board certified team of technologists focus on providing accurate and efficient care in a friendly environment. We offer routine radiology (x-ray) exams, mammography, computed tomography and ultrasound exams.
We follow The American College of Radiology (ACR) practice guidelines, as well as the radiation protections guidelines of the State of Alabama. Our mammography department is ACR accreditedand meets allMQSA standards.
CONTRACT PERIOD
Any contract resulting from this ITB will become effective upon Commission Approval.
INSTALLATION
IS EXPECTED TO BEGIN UPON COMMISSION APPROVAL AND IS EXPECTED TO BE COMPLETED WITHIN THREE (3) MONTHS BARRING ANY UNFORSEEN ISSUES. QUALITY WORKMANSHIP IS ESSENTIAL.
SCOPE OF SERVICES
Cooper Green Mercy Health Services is seeking bids from qualified bidders to supply imaging equipment for its Radiology Department as outlined below (and fully identified under the “Bid Specifications”) along with annual service agreements for equipment coverage. Pricing to include equipment removal/disposal as well as complete installation of all new equipment.

EXAMINATION OF SITE/EQUIPMENT:
All bidders are required to visit the site of the work to ascertain existing conditions. The mandatory site visit is scheduled for 9:00 A.M. on Monday, September 19, 2016 at Cooper Green Mercy Health Services, located at 1515 6th Avenue South. Please arrive 15 minutes prior to Site Tour Time so that we can be prompt with our scheduling. Failure to do so will in no way relieve the successful bidder from the necessity of furnishing all materials and equipment, and performing all work required for completion of the contract(s).
INTERPRETATIONS AND ADDENDA:
No interpretation or modification made to any respondent as to the meaning of the Bid shall be binding on the Jefferson County Commission (henceforth referred to as “the County”) or Cooper Green Mercy Health Services (Radiology Department) (henceforth referred to as the “CGMHS”) unless repeated in writing and distributed as an addendum by Jefferson County Purchasing.
