INTRODUCTION

The Athletic Trainer’s Society of New Jersey, Inc. has named two scholarships honoring outstanding students from the ATSNJ Inc. membership, who have excelled academically as an athletic training student. The first scholarship is named for Richard Malacrea. The second scholarship is the Jack Weakly District 2 Scholarship. These awards are meant to encourage the recipient to continue his/her education towards a baccalaureate degree.

The sponsoring athletic trainer is asked to nominate not more than one candidatefor each of these awards. In presenting the candidate, the sponsor must forward forms completed by the student, the team physician or an academic professor, the Head of the Department or Dean of the College/University, and the sponsoring Athletic Trainer.

The sponsor should see that all forms are completed. Forms can be downloaded and sent as an e-mail attachment to the Honors and Awards Comm. Chair with an official transcript sent separately to the address listed on the front cover. The ATSNJ Honors and Awards Committee will screen the candidates and announce its selections to the ATSNJ membership at the annual meeting. Be sure to make or save copies of this application before sending.

CRITERIA

To be eligible for consideration, an applicant shall:

  1. Distinguish oneself academically.
  1. Records of students shall be judged on their semester index at the end of their most recent spring semester.
  2. Students engaged in undergraduate programs requiring more than four years for a baccalaureate degree shall be considered for an ATSNJ scholarship during their third or fourth year.
  1. Perform with distinction as a member of the Athletic Training Student program in their declared field of study. NOTE: The degree of the student’s athletic training achievement shall be weighed at least equally with the degree of the academic performance.
  2. Signify an intention to continue academic work toward a baccalaureate degree as a full-time undergraduate student and have been judged capable of this study by the sponsoring athletic trainer, the team physician or academic professor. NOTE: the recipient of the ATSNJ scholarship is expected to remain enrolled in the undergraduate program except for military service or church obligations.
  3. Signify an intention to pursue the profession of athletic training as a career.
  4. Conduct oneself in a manner, which has brought credit to themselves, their institution and athletic training education program, and the ideals and objectives of American higher education.
  5. Must be a current member of ATSNJ Inc. at the time of application

NOTE: After satisfying the above requirements, consideration shall be given to one’s participation in campus activities other than academic and athletic training in which they have had an opportunity to demonstrate qualities of leadership and serve as an example to their fellow students.

Financial assistance from other sources shall not make the candidate ineligible for an ATSNJ award.

NOMINATING INSTRUCTIONS

  1. Nominations shall be restricted to students who are members of the ATSNJ Inc.
  2. The following forms must be completed/returned:

-Nominee application completed and signed (use NATA student member #)

-Evaluation form completed and signed (NATA member #) by the

sponsoring BOC certified athletic trainer who is a member of ATSNJ

-An endorsement to be completed and signed by the team physician or

academic professor (can be electronically sent)

-An endorsement to be completed and signed by the athletic training

program director

-Official college transcript (sent through mail to Honors and Awards

Chair)

  1. The sponsor is considered to be the responsible official for checking that the completed nomination form and related information are forwarded to the Honors and Awards Chair by the application deadline.
