Scenario
Your outstanding work on the physiological disorders signs, symptoms and diagnosis has led to your mentor assigning you with another task. She wants you to conduct some primary research into pathways and their use for individuals with physiological disorders and the strategies which can be used to support those individuals.

In order to achieve P4, you will need to plan a care pathway for each physiological disorder by contacting a care practitioner who has experience of implementing pathways within a setting of your choice linking back to one of your chosen physiological disorders

In order to achieve P5, you will need to identify and explain the care strategies that can be used in order to support individuals with one of your chosen physiological disorders.
