  1. Assessors are to insert in the ‘Rating’ column their conclusion for each requirement for compliance using the legend:

5 = Satisfactory – exceeds specified requirements.

4 = Satisfactory – complies with specified requirements.

3 = Satisfactory – observations for improvement.

2 = Satisfactory – minor nonconformances requiring action.

1 = Unsatisfactory – major nonconformances: essential that improvements be made on any future projects.

  1. Assessors are to insert in the ‘Comments’ column a summary of the basis on which the rating for each requirement for compliance is derived.
  2. Summary evidence (such as typical audit reports, NCRs, CARs, letters, and so on) in support of assessors’ comments is to be appended to the report – reference to such reports needs to be included with comments against relevant requirements for compliance.
  3. Where ‘1’ or ‘2’ ratings are recorded against any requirement for compliance, summary evidence (succinct and relevant) is to be appended to the report: similarly, it may be useful to the parties to the assessment, and/or the Asphalt Supplier Registrar, if succinct and relevant details are appended to assist in appreciation of ‘3’ ratings.
  4. Detailed documents are not to be appended to the report (for example, compliance reports, management plans) – only append extracts from such documents that are directly related to a ‘1’, ‘2’ or ‘3’ rating where such information can assist appreciation of the ratings and/or the overall report.
  5. Inclusion in the report of summary notations of relevant improvement initiatives is encouraged; for example, actions taken to implement improved quality control techniques, corrective actions that have effectively prevented recurrence of nonconformances (previously recorded with ‘1’ or ‘2’ ratings).
