PROGRAM CHANGE FORM

This form should be submitted when an institution plans to modify an approved educator preparation program from the version that was approved by the State Educator Preparation and Licensure Board (SEPLB). Changes include, but are not limited to, coursework offerings; coursework title/numbers;or status of program.

Programs must submit proposed changes 60 days prior to the proposed change taking effect. ISBE staff will review changes and make recommendation to SEPLB. SEPLB will make recommendation to State Board to accept, modify, or reject changes. (25.127[g])

REQUEST TO CHANGE AN APPROVED PROGRAM (Check All That Apply) (25.127):

25.127(i)(1)Using a format identified by the State Superintendent of Education, a State reauthorized institution may request changes to one or more currently approved programs by submitting documentation of the proposed changes no later than 60 days prior to the date upon which the changes will take effect.

25.127(i)(2)State Board staff shall review the proposed changes and make a recommendation to SEPLB, which shall submit its recommendation to the State Superintendent. The State Superintendent may accept, modify or reject any of the recommendations of SEPLB issued in accordance with this subsection (f). In cases in which the State Superintendent's modification or rejection results in an action that has negative consequences for the program, the actions to be taken shall be as described in Section 25.160.
