Application Instructions

Purpose

The Grant Application Form serves as a first screening step for potential grant activities. The form is designed to gather essential basic information about the Applicant and what it is proposing to do.

Instructions by Section

Section A: Company Profile

This section is to describe the current situation of the company and is divided into the following sections:

Item A-1: General Info - Company’s name, sector, address, telephone, and fax—self-explanatory

Item A-2: Key contact person & title—The contact person is responsible for communications between PMDP and the Applicant. This applies to all aspects of the grant, from the initial summary through completion. The contact person must have full authority and responsibility to act on behalf of the Applicant. The contact person should be someone who will be directly involved with the grant activity and has a proven, established relationship with the Applicant.

Item A-8: Consultants - Clarify if the company has ever used consultancy services and how was it paid for.

Item A-9: Pervious Donors Assistance – Inform if company benefited from other donor projects, including FNMD project and PMDP.

Item A-10:

Financial Information:

Company financial data for last three years and the projection for the following year. The financial Data is divided into two sections:

- Income Statement:

This statement shows the company’s revenues and expenses during a particular period.

- Balance Sheet:

Financial statement that summarizes a company's assets, liabilities and shareholders' equity at a specific point in time. The balance sheet equation is that total assets equals liabilities plus owner's equity.

For part time employees the question should be answered in a way that describes the average number of hours for the employee per week. For example, if a company has two part time employees. First employee works 2 full days per week, while the second employee works 5 days per week for 2 hours a day . The average will be 13 hours per week (First employee works 2*8 = 16 hours per week, second employee works 2*5 = 10 hours per week, so the average will be 13 hours (10+16= 26/2)


Section B: Project Plan

The purpose of this section is to explain the proposed plan of the company to improve their products/processes, develop new products and enter new markets

Item B-1: Project Plan Summary – list of current products that you plan to expand their markets, products that need to be improved or brand new products. On the other side, list current markets, new local and/or international markets.

Item B-2: Project Summary description – narrative description of the proposed plan.

Item B-3: PMDP Additionality - answer and provide rationale for your answer

Item B-4: Strategy and Plan to achieve objectives- This should include the needed steps to achive the objectives.

Item B-5: Breakdown of activities – fill the table for each activity, estimated cost for this activity and the duration of it.

Section C: Self – declaration

The purpose of this section is to provide a declaration from the company applied to PMDP grants that the company and its owners are in a good legal and financial status. It is also to confirm that the information provided are correct and the company is committed to inform PMDP of any changes happened to the company and its owners. On the other hand, by this declaration the company confirms that the company accepts that this application will pass PMDP evaluation process and will accept the result of this evaluation.

The applicant must carefully read and understand the Self-declaration page, then sign and stamp it.
