ALD Silent Auction Opportunity

We invite all exhibitors to take part in the annual Academy of Laser Dentistry Silent Auction, benefitting the Dr. Eugene M. Seidner Student Scholarship Program. Your product will be featured in the exhibit hall on our auction table, viewed daily by conference attendees. We offer contributing exhibitors an additional $200 advertising or sponsorship credit (upon receipt of item), sign recognition, direction to your booth during announcements whenever possible. Draw more attention to your name and products, drive more traffic to your booth, and help the education and advancement of laser dentistry – be a part of the ALD Silent Auction!

Insurance

On or before January 15, 2017, the Exhibitor agrees to provide ALD with an ORIGINAL CERTIFICATE OF INSURANCE showing the policy in effect through April 8, 2017.

Cancellation & Refund

For written or faxed cancellations received on or before January 15, 2017, a 50% refund of the total cost of booth fee will be issued. No refunds will be issued after January 15, 2017. Any exhibitors signing a contract space agreement afterJanuary 15, 2017, must pay for booth space in full and will not be eligible for a refund of any kind should they cancel their booth space agreement.

I have read and will abide by the terms and conditions on this application and in the explanation of exhibitor rules and regulations listed in this application. In accordance with the terms, conditions and regulations governing the Hilton El Conquistador Hotel, April 6-8, 2017, the undersigned hereby makes application for exhibit and workshop/seminar space(s), which application, when accepted by ALD, becomes a contract. Terms and conditions listed under Exhibitor Rules and Regulations, as well as those conditions under which exhibit and workshop/seminar space in the Hilton el Conquistador is contracted to ALD, are part of this agreement.
