  • Ensure your current school completes Section 2 and attaches the Form S1 (available from student data capture system [SDCS])
  • Follow instructions on page 3 to submit your complete application.

Interstate transfer
(from within Australia) /
  • Complete Section 1
  • Ensure your current school completes Section 2
  • Ensure your previous interstate school completes Section 3
  • Send a print-out of Section 3 with your details to the principal of your previous learning provider, to complete and return to you
  • Provide confirmation of previous enrolment and result information by attaching authenticated copies of :
­a school report or official certification of results for any senior secondary school studies (Year 11 or Year 12) completed or partially completed
­statements of attainment and/or certificates for vocational education and training (VET) and/or any other senior studies or further education which your previous school may not know about. (Conditions apply)
  • Follow instructions on page 3 to submit your complete application.

Overseas transfer

Authentication requirements

Copies of original documents should be authenticated:

You (or your carer, if you are under 16) must provide authenticated copies of required documentation.

  • Authentication can be provided by the PRINCIPAL of the receiving Queensland learning provider or by either a Commissioner for Declarations or Justice of the Peace (JP).
  • For assistance in finding a JP, visit the Department of Justice and Attorney General’s website < and select > Justices of the Peace > Finding a JP.
  • The person witnessing the declaration must write on the back of the copy: I certify that this is an accurate copy of the original document, and provide their signature and the date.

Submitting your application

  • It is your responsibility to ensure that the information on this form is complete and correct.
  • Incomplete applications cannot be processed. Applications submitted by email or fax will not be processed.

To the previous learning provider

All education authorities (including the non-government sector) have agreed to implement a national system for the transfer of student information between schools when children move from one state to another. This joint initiative — between the Australian government, state and territory education departments, and the independent and Catholic education sectors — is based on using a common "Interstate student data transfer note" (ISDTN) and set of protocols. (Ministerial Council on Education, Employment, Training and Youth Affairs [MCEETYA] 2006)
