How to submit and pay.

After you finish this application form, you can submit it by email and pay by email-transferto addition, you can pay by cash or cheque. If you pay by cheque, please mail this form along with your cheque to the following mailing address:

COMCS, 9 Legacy Landing SE, Calgary, AB T2X2E5

At this time, credit card payments are NOT accepted.

Payment Terms and Conditions

1. All students must be registered prior to attending classes at Calgary Olympic Math & Computer School.
2. Fees should be paid by email-transfer, cash, or cheque, payable to “Tsinghua School”. Please write down the name(s) of student(s) and courses registered on the Memo column. If there is no address and telephone number on the cheque, please write them down on the back of the cheque. Course fees are required to be paid in full, monthly payment options are not accepted. Cheques should be dated at the date of registration.
3. Each NSF (non-sufficient fund) cheque is subject to a $25NSF fee. Credit cards are not acceptable for now.
4. Per family, the second child registered can have a 5% discount.
5. Student scholarships have no cash value, and can only be used to pay for a course at COMCS. If you drop a course, Calgary Olympic Math & Computer School has the right to revoke scholarships awarded.
6. If a student wants to withdraw from a course, please submit a written withdrawal request two weeks before the course start date and include a receipt as proof of purchase. Tuition fees will be refunded by cheque 2 weeks upon approval of the request. Course withdrawals are subject to a $30 withdrawal fee.
7. All fees are non-refundable after three weeks from the course start date. Calgary Olympic Math & Computer School will not offer refunds for absences due to illness or personal reasons. All students are expected to adhere to the school’s code of conduct, expulsions for violations will not result in a refund.
8. Unforeseeableevents that result in reduced program hours or missed days such that the school is unable to compensate for in lecture time,or makeup days, will result in a refund in accordance with the hours missed.

Participation Waiver:

I understand that every reasonable precaution has been taken by the School to ensure the safety of the participants and the programs. I assume all risks and hazards incidental to the running of these programs and activities, and also of transportation to and from these activities, and any medical costs that may arise with respect to the participation of myself (or my kids). I do further hereby release, indemnify and hold blameless Calgary Olympic Math & Computer School, instructors and supervisors of the COMCS.

I have read and agree with the school that all fees are non-refundable three weeks after the course start date.
