Notice and Agreement
Applicants for Administrative, Teaching, or Substitute Positions
For your information, applicants for administrative or teaching assignments must sign the following document.
Toledo Public Schools (TPS) requires you to complete the following steps as part of its application process. If you are conditionally offered a position in any administrative, teaching or substitute assignment, you must complete/submit:
- A criminal records check through the Federal Bureau of Investigation (FBI) and the Ohio Bureau of Criminal Identification (BCI). This process includes the completion of fingerprint impression cards and/or authorization on the form as provided by TPS, and the payment of fees for this records check. A lesser fee may be paid if, within the past year, you have authorized BCI to send records to other Ohio school districts and you have completed the form to ask BCI to send updated records to TPS.
- In addition to providing a certified check or money order, payable to "Treasurer, State of Ohio" in the appropriate dollar amount, you will be required to complete a BCI applicant fingerprint impression card and two FBI fingerprint impression cards, and show a valid state driver's license or other satisfactory photographic identification. If the FBI and/or BCI records check indicate(s) you have been convicted of a crime and applicable law prohibits your employment, or TPS considers you to be unsatisfactory for employment because of the criminal conviction, you will not be employed. A physical examination, a tuberculin test, and a urine drug screening are necessary. The result of such examination will be used only in accordance with applicable laws. If the drug screening results are unsatisfactory to TPS, you will not be employed. Official education transcripts from the institution(s) of higher education which you attended will also be required as will a signed "Notice and Agreement" form.
- You must provide a copy of your current, valid Ohio administrative, teaching, substitute certificate, or be able to obtain one prior to employment. Also required to be submitted will be satisfactory references, a satisfactory written essay and satisfactory documentation of identification and work eligibility as required by the Department of Justice, Immigration and Naturalization Service, specifically, Employment Eligibility Verification (Form I-9). I acknowledge that if I become employed by Toledo Public Schools prior to the District's receipt and review of all required application information specified above, including but not limited to, the criminal records check, physical examination, drug screening, tuberculosis test and/or verification of identification and employment eligibility, my employment will be contingent on, and may be terminated, if any of the application information is considered unsatisfactory by Toledo Public Schools. I further acknowledge that if I falsify any employment application information, I will be subject to termination by Toledo Public Schools.
