APPENDIX ‘F’: FORM OF TENDER for CONCRETE PRODUCTS (Fillable Form, Paper Size 81/2”x 14” Landscape)

I/We, having carefully examined and read the RFT No. 1966 document, including all attachments, amendments and addenda thereto, if any, and all other related information published on the City Website and BC Bid Website, hereby acknowledges that it has understood all of the RFT document, and in response thereto hereby submits a Tender for Concrete Products.

The Form of Tender must be signed by a person authorized to sign on behalf of the Bidder and to bind the Bidder to statements made in response to this RFT and include the following:

(a)  If the Bidder is a corporation then the full name of the corporation should be included, together with the names of authorized signatories. The Tender should be executed by all of the authorized signatories or by one or more of them provided that a copy of the corporate resolution authorizing those persons to execute the Tender on behalf of the corporation is submitted;

(b)  If the Bidder is a partnership or joint venture then the name of the partnership or joint venture and the name of each partner or joint venture should be included, and each partner or joint venture should sign personally (or, if one or more person(s) have signing authority for the partnership or joint venture, the partnership or joint venture should provide evidence to the satisfaction of the City that the person(s) signing have signing authority for the partnership or joint venture). If a partner or joint venture is a corporation then such corporation should sign as indicated in subsection (a) above; or

(c)  The Bidder is an individual, including a sole proprietorship; the name of the individual should be included.

In the case of joint submissions, one party must assume overall responsibility, for communications within the RFT process and be identified accordingly in the Tender, but all parties must be jointly and severally responsible for completion of any resulting Contract.

3.  Product and Pricing for Concrete Products

(i)  Equivalent products will not be given consideration. Only approved products, as listed in the City of Nanaimo – Approved Product List, Revision November 4, 2016, Section

06.36 Service Box (Concrete) and Section 06.25 Manhole Sections will be accepted.

(ii)  Bidders are to state the Product Manufacturer, Model Offered by checking the applicable box in the Product Manufacturer and Model Offered column.

(iii)  Annual estimated quantities are an estimate only and not guarantees. The City will use these estimated quantities to determine the total estimated contract Net Value for Price Evaluation Criteria.

(iv)  Unit prices shall be firm for the initial contract term. Prices include delivery F.O.B. destination and off loading.

(v)  In case of discrepancy between unit price and its extension, the unit price will be deemed correct. Calculation errors will be corrected to reflect the accurate Net Total figure based on Unit Prices submitted. No fuel surcharge, additional levies or fees will be charged. Unit prices exclude the Provincial Sales Tax and Goods and Services Tax.

(vi)  Delivery Requirements are F.O.B. Destination: FOB Destination within City of Nanaimo limits. Deliveries for stock items will be to our Central Stores Department located at 2020 Labieux Road, Nanaimo, BC. For project orders, delivery will be to project site within the City of Nanaimo limits.

(vii)  Lead Time: The City expects delivery of the Product ordered within (48) hours after order is placed. Bidders who are unable to meet this delivery timeframe, are to provide their lead time in business days for Product delivery, in the “Lead Time In Business Days” column.

(viii)  No minimum order quantities. Products will be ordered on an “as and when required basis” in the quantities required.

(ix)  The City reserves the right to add or remove Products. Pricing will be negotiated for new Products added.

Bidders must bid all items listed in this Form of Tender for Concrete Products. Partial bids will not be given consideration.
