Please complete the scoring sheet below with the dates and number of assessments you have completed. Please complete the column names “Trainee to complete.”

A Panel will be put together to score your eportfolio and you will then be ranked. Depending on your ranking you will receive your next allocation. Please note, if you do not complete this and send it in on time, you will score lower than your colleagues and may not get the preference you want.
