Purpose

FANTA is seeking price quotations from qualified vendors that are able to provide Anthropometric measuring equipment for children and adults. This equipment will be utilized in FANTA-supported health facilities implementing nutrition assessment, counseling, and support (NACS) services in Kitwe and Mkushi Districts.

Background:

The U.S. Agency for International Development (USAID) has awarded FHI 360; a non-governmental organization based in Durham, North Carolina, the USAID Cooperative Agreement No.: AID-OAA-A-12-00005, to implement the Food and Nutrition Technical Assistance III Project (FANTA). FANTA provides technical assistance on nutrition to a number of focus countries, including Zambia.

The main objective of the FANTA NACS Acceleration Plan is to integrate NACS into routine health services at clinic and community levels through building the capacity of GRZ structures in selected districts. The specific objectives are:

•Integrate NACS into facility-based ART, PMTCT and maternal and child health (MCH) services

•Integrate NACS into community-based HBC and OVC services.

•Strengthen the capacity of service providers to provide quality NACS

Technical Specifications: Interested vendors must submit price quotations to FHI 360 for the items listed below, and that meet the following specifications:

Delivery time and Shipping:

Vendors must guarantee delivery to Lusaka, Zambia of the items with exact specifications and required quantities within 30 days from the execution of the purchase order.

Payment Schedule: All payments will be made within thirty (30) days of invoice receipt provided that items are delivered within timelines to the fullest satisfaction

Submission Requirements and Quote Format:

Please include the following in your quotations:

  • All applicable taxes and surcharges
  • Handling costs
  • Delivery timeline
  • Bank wire payment availability
  • Special USAID/Government pricing if available
  • Acceptable forms of payments
  • Cancellation policies

Do not include shipping costs. Shipping costs will be covered directly by FHI 360.

RESPONSE deadline:

-Responses should be submitted by email to Zabi Girowal () by August 3, 2016 4:30 PM (business working hours US Washington DC). Any quotes received after this deadline may not be accepted and shall be considered non-responsive. FHI 360 will acknowledge receipt of the quotation by return email.

-Any questions or requests for clarification need to be submitted in writing to the same email address as above by August 2, 2016 3:00 PM. Inquiries and answers to inquiries will be shared with all applicants. No telephone inquiries will be answered.

All materials produced and delivered by the vendor, and all work products, will be considered “work for hire” and will become the exclusive property of FHI 360. The vendor must be prepared to warrant that all deliverables will be free of material faults and errors and that no copyrights have been violated.

Minimum Requirements:

Fulfillment of all Specifications and requirements listed above.

Offer Acceptance Period

Submitted quotes should be valid for sixty (60) days from the day of submission to allow adequate time for organizational approval.

Evaluation Criteria:

Only vendors that meet all the requirements as stated above will be considered. Selected vendor will be based on lowest price that meets all the requirements.

Payment Terms:

Quotes should include payment terms.

Contracting Mechanism:

A fixed-price purchase order will be awarded to the responsive vendor whose quote will be evaluated as the lowest in price. In the event that equipment and supply amounts need to be shifted during the time period between the signature of a purchase order with the selected vendor and the actual event, FHI 360 will modify the purchase order

NOTE

1) FHI 360 will not compensate vendors for preparation of their response to this RFQ.
2) Issuing this RFQ is not a guarantee that FHI 360 will award a purchase order.
3) FHI 360 reserves the right to issue a purchase order based on the initial evaluation of offers without discussion.
4) FHI 360 may choose to award a purchase order for part of the activities in the RFQ.
5) FHI 360 may choose to award a purchase order to more than one vendor for specific goods in the RFQ.
6) FHI 360 may request from short-listed vendors a second or third round of written response to a more specific and detailed scope of work that is based on a general scope of work in the original RFQ.
7) FHI 360 has the right to rescind an RFQ, or rescind an award prior to the signing of a purchase order due to any unforeseen changes in the direction of FHI 360’s client (the U.S. Government), be it funding or programmatic.

8) FHI 360 reserves the right to waive any deviations by vendors from the requirements of this solicitation that in FHI 360’s opinion is considered not to be material defects requiring rejection or disqualification; or where such a waiver will promote increased competition.

REQUEST FOR PROPOSAL FIRM GUARANTEE

All information submitted in connection with this RFQ will be valid for forty (60) days from the RFQ due date. This includes, but is not limited to, cost, pricing, terms and conditions, service levels, and all other information. If your firm is awarded the contract, all information in the RFQ and negotiation process is contractually binding.

OFFER VERIFICATION

FHI 360 may contact offerors to confirm validity of the offer, contact person, address, and bid amount.

FALSE STATEMENTS IN OFFER

Offerors must provide full, accurate, and complete information as required by this solicitation.

CONFLICT OF INTEREST

Offerors must provide disclosure of any past, present, or future relationships with any parties associated with the issuance, review, or management of this solicitation and anticipated award. Failure to provide full and open disclosure may result in FHI 360 having to re-evaluate selection of a potential offeror.
