Annual StudentandParent Verification Form 2016-17
The Macon County R-1 School District seeks to inform students and parents of district policies and student handbook rules by providing access to our student handbooks (online at to each student/parent. If a student doesn’t have access to the online version of the student handbook he/she will be given a paper copy of the student handbook if requested by the student or their parent. Students and parents are strongly encouraged to read the student handbook. Should you have questions, contact your building administrator. Included in the student handbook, you will find the District Technology Policy, Student Discipline Code and the Citizenship Policy. By signing this form a student is stating he/she has read the student handbook and understands the information contained in it. Students and parents are REQUIRED to sign and return this form before the student may use district technology, i.e. computers, copiers, calculators, and other handheld devices.
(For Students and Parents): I have read the school district’s Technology Usage Policy, administrative regulations and netiquette guidelines and agree to abide by their provisions. I understand that violations of these provisions may result in disciplinary action, including, but not limited to suspension or revocation of access to district technology and suspension or expulsion from school.
I understand that use of the district’s technology is not private and that the school district may monitor all use of district technology, including but not limited to accessing browser logs, e-mail logs and any other history of use. I consent to district interception of or access to all communications sent, received or stored using the district’s technology resources, pursuant to state and federal law, even if the district’s technology resources are accessed remotely.
(For Parents Only): I agree to be responsible for all costs or damages incurred from the use of district technology by my child or a child under my care.
***By signing and returning this verification form, students and parents acknowledge they have had access to the student handbook and read the district’s Student Discipline Code and Citizenship Policy.
