InaccordancewiththeSENCodeofPractice,allStatementsofSEN/Education,HealthandCarePlans(EHCPlans)mustbereviewedatleastannually.

TheAnnualReviewisavitalpartoftheacademicyear,asanopportunitytoensurethesupportdetailedintheStatement/EHCPlanhasbeenprovided,andremainsappropriate.Itisalsoaforuminwhichparents/carers,educational/trainingsettingsandpupilscanraiseanyconcernsregardingthesupportorplacement,andhavetheseaddressedwhereverpossible.

Ofequalimportance,theannualreviewisanopportunitytocelebrateapupil'ssuccessesthroughouttheyear.

TheAnnualReviewshouldbeperson-centred,takingintoconsiderationtheneedsofboththepupilandtheirparent(s)/carer(s).Educational/trainingsettingshavetheflexibilitytoadapttheirannualreviewprocessinordertomeettheseneeds,butmustensurethattheAnnualReviewReportForm iscompletedthoroughly.

BeforetheAnnualReview

TheSENTeamissuesAnnualReviewplanningreportsatthebeginningofeachacademicyear,detailingthedatebywhicheachStatement/EHCPlanmustbereviewed.Educational/trainingsettingsshouldplantheirAnnualReviewsfortheacademicyearinaccordancewiththesedates.

AllprofessionalsinvolvedwithapupilshouldbeinvitedtotheAnnualReview(andencouragedtoprovideawrittenreportifappropriate).Ifitisfeltthatanupdatedprofessionalreportisnecessary

(e.g.anupdatedSALTreport,furtherassessmentofapupil'sskillsbyanEducationalPsychologist),thismustberequestedbytheeducational/trainingsettingearlyenoughtoensurethatacompletedreportisavailablefortheAnnualReview.

AdraftAnnualReviewReportForm shouldbeissuedtoallthoseinvitedtothemeetingatleasttwoweekspriortotheAnnualReviewmeeting,accompaniedbyanyrelevantreports(e.g.EducationalPsychologistRecordofContact,SALTreport,Physiotherapy/OccupationalTherapyreport).Thisallowsindividualstheopportunitytoreadthrougheachreportpriortothemeeting,andprepareanyresponses.

CompletingtheAnnualReviewReportForm

TheAnnualReviewReportFormistheformalrecordofapupil'sprogressoverthepreviousyear.Itmaybeusedtoinform anAmendmenttotheStatement/MovingOnPlan/EHCPlan;supportarequestforchangeofplacementoradditionallevelofsupport;or,insomecircumstances,besubmittedasevidencetowardsaTribunal.Assuch,theformmustbecompletedthoroughlyandaccurately.

AftertheAnnualReviewMeeting

Allannualreviewpaperwork(includingsupportingdocumentation)mustbesubmittedtotheLocalAuthoritynolaterthantwoweeksafterthemeeting.TheLocalAuthoritywillthenrespondtothereviewaccordingly,andisrequiredtodosowithintwoweeks.
