General Instructions:

1.  Please download the Annual Report Template to your computer for completion.

2.  Annual reports must follow the exact format of the Annual Report Template.

3.  Supporting charts, tables or other exhibits should be inserted in the appendix section.

4.  All administrative department annual reports are to be submitted by e-mail attachment to the appropriate Chief Administrative Officer for your unit and to Office of Institutional Research and Assessment at

The strategic planning goals for the next three years, including linkages to the University’s strategic goals, identified funding sources and priorities, time tables, and assigned responsibilities:

A summary of planning objectives for the coming academic year:

Brief plan and schedule for the next external review of the department:


4. OUTCOMES

Notable events concerning the department (accreditation, external reviews, new programs and related information):

Review and revise the Administrative Unit Assessment Plan for the department and attach the revised plan in the appendix.

Provide a summary of the outcomes from surveys, benchmarks for action, and other assessment metrics from your programs and services. Include data pertinent to the operations of the department, e.g., fundraising trend data, benchmark or other comparative data, etc.:
