Andover Central Middle School
Description: This camp is a skills camp to help young ladies become better volleyball players. It will focus on the skills of passing, hitting, serving, etc. to help the athletes learn important techniques to become successful volleyball players.
The coaches of Andover Central Middle School will assist in teaching the skills by demonstrating drills and techniques.

**Please do not hand money into the office. I do not work within the district and will not receive your information or payment. All necessary paperwork must be mailed to the address listed above.
I, ______, being of the parent or legal guardian of ______, a minor child, do hereby forever release Andover Central Middle School, USD 385, and its coaches from any and all liability for personal injuries which might be suffered by my child, or property damage which might be incurred, while said child is enrolled in, and participating in, a volleyball camp which will be conducted by said School District in 2016. This release shall apply in cases of said child being overexerted or injured or, in any manner being harmed, during said volleyball camp or on the school premises before or after said camp.
I further state that said child is in good physical health, and I have had her examined by a physician who has released said child to participate fully in said camp. I have not withheld any physical ailments or health defects of said child from the camp personnel. I realize that I have the ultimate responsibility for said child and I know and realize the physical stresses or rigors of participating in said camp. I realize I have the right at any time to forbid said child from participating in said camp, or from any specific portion of said camp and I agree to divulge any health problems that said child may have, to the camp personnel or school administration.
I have full authority to sign this release and to bind said child and myself by my signature hereto.
