To improve health outcome for our clients

Community Need:

Providing individual and group level health education, risk reduction counseling, supportive counseling, HIV/Hepatitis C testing, and awareness to people who inject or use drugs, and at risk for HIV and/or Hepatitis C. By providing health and hepatitis care coordination through harm reduction, people are likely to engage and retain and take ownership of his/her health. In addition, holistic services such as acupuncture provide people are given an opportunity to engage more in their health care.

Member Position Summary:

The Referral Specialist is responsible for coordinating and completing referrals ordered by FQHC providers, acquiring and verifying third party information for referrals, obtaining and arranging referral appointments, tracking referrals.

Familiarity with EMR (eClinical Works)

Member Impact:

The impact of the position will increase communication and participation in our Hep C initiative that will assist clients with improving health outcomes.

Essential Functions of Position:

I. Performs Administrative and Clinical Support Activities

a. Responsible for day-to-day coordination of patient screening activities including the tracking of Hepatitis C treatment eligible patients

b. Gathers information needed to obtain Prior Authorization for Hepatitis C medication treatment.

d. Compiles data needed for Prior Authorization Appeals to patient’s insurance.

e. Work in collaboration with the Patient Navigator/Health Educator.

II. Coordinates Communication and Hepatitis C Health Education

a. Provides health education and patient instructions as defined by protocol and as appropriate to promote the understanding and adherence to Hepatitis C testing.

b. Provides sensitive and accurate information to patients, including those tested for

d. Consistently utilize and facilitate effective strategies to communicate pertinent information in a timely manner.

e. Adhere to privacy and information security requirements (HIPAA) and other regulations. Also performs functions, duties and/or services in compliance with regulatory agencies, contractual obligations and funding source requirements.

f. Through team work, active participation, and accountability, exhibits behaviors and attitudes of courtesy and respect for all members of the Work unit in accordance with Clinic’s mission and values.

g. Maintains high level of respect, dignity and confidentiality in handling patient information at all times without being judgmental.

h. Work collaboratively with other departments. Promotes and support the patient centered medical home.

Member Prohibited/Unallowable Activities & Rules of Conduct:

  1. Federal AmeriCorps Member Prohibited Activities
  2. While charging time to the AmeriCorps program, accumulating service or training hours, or otherwise performing activities supported by the AmeriCorps program or the Corporation, staff and members may not engage in the following activities:
  3. Attempting to influence legislation;
  4. Organizing or engaging in protests, petitions, boycotts, or strikes;
  5. Assisting, promoting, or deterring union organizing;
  6. Impairing existing contracts for services or collective bargaining agreements;
  7. Engaging in partisan political activities, or other activities designed to influence the outcome of an election to any public office;
  8. Participating in, or endorsing, events or activities that are likely to include advocacy for or against political parties, political platforms, political candidates, proposed legislation, or elected officials;
  9. Engaging in religious instruction, conducting worship services, providing instruction as part of a program that includes mandatory religious instruction or worship, constructing or operating facilities devoted to religious instruction or worship, maintaining facilities primarily or inherently devoted to religious instruction or worship, or engaging in any form of religious proselytization;
  10. Providing a direct benefit to-
  11. A business organized for profit;
  12. A labor union;
  13. A partisan political organization;
  14. A nonprofit organization that fails to comply with the restrictions contained in section 501(c)(3) of the Internal Revenue Code of 1986 except that nothing in this section shall be construed to prevent participants from engaging in advocacy activities undertaken at their own initiative; and
  15. An organization engaged in the religious activities described in paragraph (vii) of this section, unless Corporation assistance is not used to support those religious activities;
  16. Conducting a voter registration drive or using Corporation funds to conduct a voter registration drive;
  17. Providing abortion services or referrals for receipt of such services; and
  18. Such other activities as the Corporation may prohibit.

AmeriCorps members may not engage in the above activities directly or indirectly by recruiting, training, or managing others for the primary purpose of engaging in one of the activities listed above. Individuals may exercise their rights as private citizens and may participate in the activities listed above on their initiative, on non-AmeriCorps time, and using non-CNCS funds. Individuals should not wear the AmeriCorps logo while doing so.

  1. Unallowable Activities:
  2. An AmeriCorps member may spend no more than ten percent of his or her originally agreed-upon term of service, as reflected in the member enrollment in the National Service Trust, performing fundraising activities, as described in §2520.40. AmeriCorps members may participate in fundraising activities but ONLY under limited circumstances:
  3. Members may generate funds/resources directly in support of the AmeriCorps program’s service activities. Examples of fundraising activities members may perform include, but are not limited to the following:
  4. Seeking donations of books from companies and individuals for a program in which volunteers tutor children to read;
  5. Writing a grant proposal to a foundation to secure resources to support the training of volunteers;
  6. Securing supplies and equipment from the community to enable volunteers to help build houses for low-income individuals;
  7. Securing financial resources from the community to assist in launching or expanding a program that provides social services to the members of the community and is delivered, in whole or in part, through the members of a community-based organization;
  8. Seeking a donation from alumni of the program for specific service projects being performed by current members.
  9. AmeriCorps members may not:
  10. Raise funds for living allowances or for an organization's general (as opposed to project) operating expenses or endowment;
  11. Write a grant application to the Corporation or to any other Federal agency.
  12. Nonduplication. Corporation assistance may not be used to duplicate an activity that is already available in the locality of a program. And, unless the requirements of paragraph (f) of this section are met, Corporation assistance will not be provided to a private nonprofit entity to conduct activities that are the same or substantially equivalent to activities provided by a State or local government agency in which such entity resides.
  13. Nondisplacement.
  14. An employer may not displace an employee or position, including partial displacement such as reduction in hours, wages, or employment benefits, as a result of the use by such employer of a participant in a program receiving Corporation assistance.
  15. An organization may not displace a volunteer by using a participant in a program receiving Corporation assistance.
  16. A service opportunity will not be created under this chapter that will infringe in any manner on the promotional opportunity of an employed individual.
  17. A participant in a program receiving Corporation assistance may not perform any services or duties or engage in activities that would otherwise be performed by an employee as part of the assigned duties of such employee.
  18. A participant in any program receiving assistance under this chapter may not perform any services or duties, or engage in activities, that—
  19. Will supplant the hiring of employed workers; or
  20. Are services, duties, or activities with respect to which an individual has recall rights pursuant to a collective bargaining agreement or applicable personnel procedures.
  21. A participant in any program receiving assistance under this chapter may not perform services or duties that have been performed by or were assigned to any—
  22. Presently employed worker;
  23. Employee who recently resigned or was discharged;
  24. Employee who is subject to a reduction in force or who has recall rights pursuant to a collective bargaining agreement or applicable personnel procedures;
  25. Employee who is on leave (terminal, temporary, vacation, emergency, or sick); or
  26. Employee who is on strike or who is being locked out.
  1. Rules of Conduct:
  1. The member is expected to, at all times while acting in an official capacity as an AmeriCorps member:
  2. Comply with the rules and standards of the host agency;
  3. Demonstrate mutual respect toward others;
  4. Demonstrate professional, courteous conduct;
  5. Follow directions
  6. Attend service trainings and activities on time and ready to serve and learn;
  7. Maintain a physical appearance reflective of his/her role in the AmeriCorps Program;
  8. Direct concerns, problems, and suggestions to his/her assigned Host Site Supervisor.
  1. The member understands that the following acts also constitute a violation of the Program’s rules of conduct:

i.Unauthorized tardiness

ii.Unauthorized absences

iii.Repeated use of inappropriate language (i.e., profanity) at a service site

iv.Failure to wear appropriate clothing to service assignments

v.Stealing or lying

vi.Engaging in any activity that may physically or emotionally damage other members of the program or people in the community

vii.Unlawful manufacture, distribution, dispensation, possession, or use of any controlled substance or illegal drugs during the term of service

viii.Consuming alcoholic beverages during the performance of service activities

ix.Being under the influence of alcohol or any illegal drugs during the performance of service activities

x.Failure to notify the program of any criminal arrest or conviction that occurs during the term of service

  1. Under the Drug-Free Workplace Act, a member must immediately notify their Host Site Supervisor and Program Director if they are convicted under any criminal drug statute. Member participation in the Program is conditioned upon compliance with this notice requirement, and the grantee may take action for violation of this.
  2. In general, for violating the above stated rules, the Program will do the following (except in cases where during the term of service the member has been charged with or convicted of a violent felony, possession, sale, or distribution of a controlled substance): For the member’s first offense, the Program will issue a verbal warning. For the second offense the Program will issue a written warning. Should the offense continue, the program may extend another written warning or exit te member from the program. The program will notify the member of any violation and action being taken in writing. Notification signed by both the member and program supervisor will be kept in the member’s file.
  3. The member understands that he/she will be either suspended or released for cause in accordance with above paragraphs of this agreement for committing certain acts during the term of service..

D. Members may be involved in direct patient contact and /or relations. In order for the health center to provide complete health services, patients are often asked to reveal personal information, financial data, and health status. To ensure patient dignity and privacy, CHCANYS Healthy Futures Corps insists that members do not communicate patient personal, financial, or medical information to anyone both inside and outside of the program site. If anyone outside of the program requests information regarding patients, immediately notify your Program Coordinator or Site Supervisor, who will determine the proper course of action. Breaching the confidentiality of a patient’s records and/or information may result in service reassignment and/or immediate removal from the program (without eligibility for a pro-rated education award).

Required Knowledge, Skills, and Abilities:

1. Demonstrate ability to work effectively independently and as part of a team, in collaborative settings, required.

2. Excellent written and verbal communication skills required. Understands and is committed to maintaining highest level of confidentiality.

3. Demonstrated ability to build the trust and respect of patients, staff, colleagues, external contacts, and other TVHC members.

4. Excellent problem-solving skills required, including creativity, resourcefulness, timeliness, and technical knowledge related to analyzing and resolving medical problems.

5. Sensitivity to needs of culturally and linguistically diverse patient and employee population.

7. Demonstrated ability to meet or exceed the Service Excellence Standards of TVHC, Inc.

8. Willingness and ability to work some evenings and weekends, as needed.

9. Demonstrated ability to utilize computer technology and to willingness to develop and adapt to the evolving technological requirements of modern medical health center practices required.

Required Academic and Experience Qualifications:

1. Degree in Public Health or equivalent in experience is required.

2. Prefer minimum three years of experience in project or program management, preferably in medically needy and underserved communities and in a community health center setting.

3. Previous experience providing services and exercising leadership in a culturally and linguistically diverse setting, with demonstrated success.

4. Previous experience working within a managed care environment strongly preferred.
