QUESTIONS ABOUT PHYSICAL ACTIVITY. The next questions are about the frequency and type of physical activity you do. EXERCISE is defined here as planned, structured, and repetitive physical activity done to promote health and fitness (e.g., walking, jogging, stair climbing, aerobics, cycling, rowing, swimming, weight training, etc.). SPORTS is defined here as physical activity requiring effort and skill and played according to rules. UNIVERSITY SPORTS means being a part of an organized team or club at W.U.. INFORMAL SPORTS are sports played as part of an intramural team, event, or social sports league (e.g., city/church league softball) or during your free time with others (e.g., pick up basketball or ultimate Frisbee). The WORK category is for mild, moderate, or strenuous activities you do as part of a volunteer or paid job.

Considering a 7-DAY period (a week), how many TIMES on average do you do the kinds of physical activity below for more than 15 minutes? Please write the number of times per week in the box for each type of activity.
