HEALTH FORM
YL Health Form is due by Friday, May 1st, 2015
Every parent/guardian must complete the YL Health, Consent & Release Form(YL Health, Consent & Release) for their camper. The form must be signed by a parent/guardian in several places, and the physical exam portion of the formmust be completed and signed by a Physician. If a doctor has not seen your child within the last year, your child must complete their annual physical by May 1st, 2015. The shot record must also be written onto the health form, not attached.
DEPARTURE & ARRIVAL TIMES
Departure: Saturday, July 18th @ 6:00pm from Dave & Busters parking lot (4214 Capital of TX Hwy N). Please arrive by 5:30pm to check in and find a seat on the bus
Return: Sunday, July 26tharound 2:00pm in the Dave & Busters parking lot. Once the bus leaves Crooked Creek we will communicate with parents a more specific time of arrival
**Please send your camper with money for two meals on the road. The bus will stop once each way on the way TO and FROM camp.

**It is important to bring clothes to camp, but not your entire wardrobe!! Please limit your luggage to one large bag and one carry-on bag.
**All medicines, including over the counter items, must be placed in a zip-lock bag with your student’s name written on it. We will collect these at the bus before departure.
IMPORTANT NOTICES
- Young Life prohibits the use of illegal drugs and alcohol, anyone choosing to bring such items will be sent home at his/her own expense. Leaders reserve the right to look through belongings if there is any suspicion. The Camp Director will deal with any disruptive behavior and students may be sent home at his/her own expense if there is no compliance.
- Your IPODS/Cell Phones are welcome on our bus trip, but will not be allowed out on the trail and wouldn’t have any reception anyway. Please write your name on them and they will be collected before we enter the camp. Camp phones are available for students to call home and trail guides will have an emergency telephone as well.
