Job Title:FallsCo-ordinator

Job Location:Sharples Building, Church Road, Urmston

Responsible to:Care Services Manager

Hours of work: 30 hours per week Monday to Friday

(to suit operational needs)

Salary: Scale point 26: £22,937 pa

JOB SUMMARY

To Co-ordinate a Trafford Falls Prevention Exercise Programme, including people with dementia.

KEY TASKS AND RESPONSIBILITIES

1.1 Co-ordinate and develop a Traffordwide Falls Prevention Exerciseprogramme

1.2 Where appropriate arrange registration and attendance at Falls exercise sessions.

1.3 To liaise with professionals to identify and facilitate progression of clients to follow-on schemes.

1.4 To liaise with Leisure Trust and private providers of exercise classes to develop an appropriate referral system based on customer need.

1.5 To liaise with residential and nursing homes to develop regular strength and balance exercise programmes for residents

1.6 Recruit and train volunteers to support the overall development and also individual exercise classes

1.7 Attend Falls Prevention group meetings as directed.

1.8Deliver specified Age UK Trafford falls prevention events

1.9 Assist with Falls Prevention events in the Borough.

1.10Support the development of a Borough wide Falls prevention programme creating a better system between health/GP's/housing agencies/social care/leisure and private exercise providers.

1.11To research and keep abreast of developments in relevant assistive technology and introduce into Age UK Trafford’s resources as appropriate

1.12Using the organisation’s database, to provide reports and statistics on a regular basis.

1.13 To participate in all evaluation and monitoring procedures.

1.14To attend briefings/staff meetings and training sessions.

1.15 To carry out all duties in compliance with current Health and Safety Legislation and Age UK Trafford’s Health and Safety policies.

1.16 To carry out all duties in compliance with current Age UK Trafford’s policies,ensuring the provisions of equal opportunities.

1.17 Such other duties as may be required and are consistent with the duties and responsibilities of the post.

2. Outcomes

The postholder will be required to support the Chief Executive of Age UK Trafford demonstrate the following outcomes as part of the overall Health & Wellbeing Contract:

2.1 Medium Term:

  • Improved awareness of falls prevention in the older population of Trafford

2.2 Longer Term:

  • A reduction in falls in the older population of Trafford
  • A reduction in both initial hospital admissions and readmissions for falls in Trafford
