HUH Advance Management Resident

(i.e. ER/IM HUH)

Expectations:

  • Arrive at 7AM in 7 Brush lounge
  • Assign the admission pager (0092) to yourself
  • Admit patients to the on-call team
  • When the ED calls you for an admission please take sign-out, briefly review the chart, ask the ED physician any questions you might have, provide the attending’s name and go evaluate the patient immediately or at least within 15 minutes.
  • Place the following orders:
  • Covering physician order
  • Basic orders until the primary team evaluates the patient (which should be after morning report)
  • Short-call team should begin carrying the admission pager at 1 (until 3 pm long call team)
  • Sundays are your days off

When Seniors are Offor in Clinic (Med-Peds seniors):

  • You are expected to round with the team who has a senior off
  1. Receive brief sign-out from that senior the evening before
  2. Pre-round with the team’s interns and students (and communicate that time and location with them so that they can be prepared)
  3. Staff team questions with the interns and medical students throughout the day
  4. This mean that you need to stay and make sure that the interns see their patients, staff with them, and make sure that all orders are placed properly and go over the sign out on those patients with them
  5. Provide sign-out to the on-call senior and to the team’s senior who is off after your shift
  6. Make sure your interns/students attend morning report
  7. You are NOT responsible for writing notes (except those which are written by Sub-I’s which require a senior resident note as well)unless you are covering for an on-call senior which needs to be approved by the CMR and attending physician
  • Only after finishing all those tasks should you leave the hospital
  • You can still transfer the admission pager to the short-call resident at 1 pmbut must remain in-house to support the interns who’s senior is off until they sign out their patients (at or after 3 pm)
  • AT ANY POINT IN TIME PATIENTS CAN DECOMPENSATE OR ADMISSIONS CAN INCREASE. WE NEED TO ENSURE SUFFICIENT COVERAGE OF THE FLOORS BY SENIOR RESIDENTS, INCLUDING OUR ER/IM SENIOR RESIDENT
  • If the resident you are covering is in clinic please give them a brief sign out on those patients after he/ she is back from clinic

**PLEASE NOTE: Since you will still be carrying the admission pager you may need to break away from the team to admit the patient (as above) then rejoin the team on rounds and then on the floors to answer questions and help with patient care
