TARGETED SPORTS PROGRAM EXPRESSION OF INTEREST AND APPLICATION FORM

Dear Parents and Students,

Thank you for your interest in the Targeted Sports Program at our school for 2014. Students participating in the Targeted Sports Program undertake specialised development programs in their chosen sport during school time as a school subject. Touch Football has been targeted in 2014, with the plan to include more sports starting in 2015. Touch Football will be offered for both males and females. This is integrated with a full academic curriculum which accommodates for the special needs of the targeted sports athlete. The Targeted Sports Program will accept up to 20 placements in Touch Football and will only be available to students in years 7, 8, 9 and 10 in 2014. The program will run for 3 Terms starting Term 2, Week 1.

PLACEMENT IN THE PROGRAM IS DEPENDENT ON:

• An ability, interest and potential in the nominated sport.

• A recognisable commitment to school studies and academic performance.

• An excellent record of school conduct and attendance.

CONTINUATION IN THE PROGRAM IS DEPENDENT ON:

• Continued development and improvement in the nominated sport, satisfactory academic performance, acceptable levels of attendance, payment of TSP fees and adherence to the schools’ code of conduct.

Please note the following sequence required to gain placement in the program:

  1. Complete “TARGETED SPORTS PROGRAM APPLICATION FORM”
  2. Return form to the PDHPE faculty.
  3. Applicants suitability for the program will be based upon set criteria; Attendance, General Behaviour, Punctuality, Attitude and the wearing of School Uniform. A panel made up of staff from Oxley High School will review each application and make a decision of those students that will be allowed to trial.
  4. Successful and unsuccessful applicants will be advised in writing. This process can take up to 4 weeks from trials.

• Please ensure information you are submitting is true and correct. If it is found to be incorrect any offer for placement may be withdrawn. All information is treated with strictest confidence.

It is compulsory that Targeted Sports Program students compete to the best of their ability at school Athletic, Cross Country and Swimming Carnivals and represent the school at these fixtures.

MEDICAL CONDITION:

To the best of my knowledge, the student applying has no medical condition, physical disability or injury which puts him / her at risk when participating in the school's sporting programs.

The Touch Football program will attract a fee of $250,which must be paid for in full for the student to be accepted in the program. This fee includes bus fees, hiring of Gipp Street Touch Fields and the use of swimming pools and local gyms to add variety to the program.

I / We accept that there will be selection trials for placement and continuation in the Targeted Sports Program in the years to come, and agree to pay the charges associated with the school's sporting programs.
